Estado nutricional, função pulmonar e força muscular respiratória de indivíduos em hemodiálise submetidos à cinesioterapia respiratória

Resumo: Individuals on hemodialysis show a reduction of muscle tissue which contributes to deficits in their activities of daily living and the ability to perform exercises. Given these factors and the possibility of changes in respiratory system is imminent. This study aimed to determine the nutritional status, to compare before and after a program of respiratory exercises the hematologicals and biochemicals parameters, muscle strength, lung function, changes in trunk volume and physical aspects related-self to quality of life. Nutritional status was determined by 24-hour recall (three days, one day of weekend), biochemical and hematological parameters (hematocrit, hemoglobin, total lymphocytes, urea, creatinine, albumin) and anthropometric measurements, triceps skinfold (TSF) and arm muscle circumference (AMC). Respiratory muscle strength was estimated by MIP (maximal inspiratory pressure) and MEP (maximal expiratory pressure). Lung function was assessed by the spirometric variables, FVC (forced vital capacity), FEV1 (end-expiratory volume in one second) and PEF (peak expiratory flow). For the kinematic analysis, was used 30 markers fixed on the trunk of the individuals. This, was divided at four compartments U-C (upper chest), L-C (lower chest), U-A (upper abdomen) and L-A (lower abdomen). Nutritional data were described as mean and standard deviation. For statistical analysis were used t student test and Spearman correlation coefficient. The nutritional profile detected losses in protein-energy consumption of these individuals. Serum concentrations of calcium and iron were below forecast in some individuals while consumption salt remained within the expected range for this population (1-3 g / day). Anemia was present in most individuals. Creatinine concentration was lower than recommended in most individuals. Anthropometric measurements showed a great variability among investigators when it was considered the age.The program of respiratory exercise improved scores on all dimensions of KDQOL- SF, a significant improvement in MIP, FVC and FEV1 (forced expiratory volume in one second) showing the effects of this therapy in improving muscle strength and pulmonary function in this population. Kinematic analysis showed that the volumes and their variations after exercises respiratory reflected an improvement in use of respiratory muscles. With regard to abdominal muscles, the activation of your muscle fibers have been small.

Diante destes fatores, a possibilidade de alterações no sistema respiratório é iminente. Este estudo teve por finalidade determinar o perfil nutricional, comparar pré e pós programa de cinesioterapia respiratória (CR) os parâmetros hematológicos e bioquímicos, força muscular respiratória, função pulmonar, variações de volume de tronco e aspectos físicos relacionados à qualidade de vida. O estado nutricional foi determinado pelo recordatório de 24 h (três dias, sendo um dia do final de semana), parâmetros hematológicos e bioquímicos (hematócrito, hemoglobina, linfócitos totais, uréia, albumina e creatinina e as medidas antropométricas, prega cutânea tricipital (PCT) e circunferência muscular de braço (CMB). A força muscular respiratória foi estimada pela Pimáx (pressão inspiratória máxima) e Pemáx (pressão expiratória máxima). A função pulmonar foi avaliada por meio das variáveis espirométricas, CVF (capacidade vital forçada), VEF1 (volume expiratório final no primeiro segundo) e PFE (pico de fluxo espiratório). Para a análise cinemática foi fixado 30 marcadores no tronco dos indivíduos que o dividiu em 4 compartimentos TX-S (tórax superior), TX-I (tórax inferior), AB-S (abdome superior) e AB-I (abdome inferior). Os dados nutricionais foram descritos como média e desvio padrão. Para análise estatística foram utilizados o teste t de Student e o coeficiente de correlação de Spearman. O perfil nutricional detectou prejuízos no consumo energético-protéico destes indivíduos. As concentrações séricas de cálcio e ferro estiveram abaixo do previsto em alguns indivíduos enquanto o consumo de sal permaneceu dentro do esperado para esta população (1-3 g/dia). A anemia esteve presente na maioria dos indivíduos. A concentração de creatinina foi inferior ao recomendado na maioria dos indivíduos. As medidas antropométricas apresentaram grande variabilidade quando comparados com outros estudos levando em consideração a idade. Este fato pode ser explicado pela variação de medidas entre observador e também com medições repetidas realizadas por um único observador. Vale ressaltar que o número pequeno da população possibilitou a análise de muitas variáveis e pôde oferecer uma avaliação detalhada do estado nutricional destes indivíduos, contribuindo para o melhor suporte oferecido a esta população. O programa de cinesioterapia respiratória melhorou os escores em todas as dimensões do KDQOL-SF, promoveu uma melhora significativa na Pimáx e nos valores de CVF (capacidade vital forçada) e VEF1(volume expiratório forçado no primeiro segundo) comprovando os efeitos desta terapia na melhora da força muscular e da função pulmonar da população estudada. A análise cinemática mostrou que os volumes e suas variações obtidas após a CR refletiram uma melhora no uso da musculatura inspiratória.
