Avaliação da capacidade funcional, da dispneia e da qualidade de vida de pacientes com doença pulmonar obstrutiva crônica participantes de um programa de reabilitação pulmonar

Resumo: The Chronic Obstructive Pulmonary Disease (COPD) is a preventable and treatable respiratory disease, which is characterized by the presence of chronic airflow obstruction which is not completely reversible after the bronchodilator use. The airflow limitation is usually progressive and it is associated with an abnormal inflammatory response of the lungs to inhaled particles or toxic gases, caused primarily by smoking. It is one of the mainly causes of morbidity and mortality in the whole world. The deterioration of the lung function throughout the time, associated with the systemic effects which lead to a permanent disability, is evidenced by fatigue, exercise intolerance and consequent worsening of the fitness which limits the daily activities, with reduced on the quality of life. Besides that, dyspnea contributes significantly to reduced a worse prognosis of the disease. The pulmonary rehabilitation, if focused on physical exercises, as documented in literature, has been considered the most effective alternative to minimize the deleterious effects of deconditioning in these patients. Objective: evaluate the program pulmonary rehabilitaton on functional capacity, on dyspnea index and on its impact on the quality of life of the patients with COPD. Methodology: It is test clinic not control which was approved by the HC-UFG ethics committee on research. Seventeen patients, who participated in the Pulmonary Rehabilitation Program, with COPD were evaluated at the time of the admission to the program and also after 20 training sessions. The patients with musculoskeletal diseases who limited walking or were showing significant cognitive impairment were excluded. The analyzed variables were age, sex, degree of pulmonary obstruction (VEF1), body mass index (BMI), respiratory muscle strength, BODE index of mortality, quality of life by the St. George's Respiratory Questionnaire (SGRQ), the impact of dyspnea during the daily activities analyzed in the scale assessed by the London Chest Activity of Daily Living (LCADL), dyspnea index (MRC) and the distance walked during the six-minute walk (6minWD). Statistical Analysis: To test the normality of these data, the Kolgomorov-Smirnov test was used; to compare the variables pareades before and after the pulmonary rehabilitation the Student t test was used and to value the association between the variables was used the coefficient of correlation of Pearson. RESULTS: Were evaluated 17 patients, mean age of 72.46 ± 5.84 years old, FEV1% pred. 48.54 + 11.14, BMI 23/89 +2.80 kg/m2. There was a improvement in the test six-minute walk (p = 0.001) and there was an improvement in the inspiratory muscle strength (p= 0.042), there was reduced of dyspnea (p = 0.002), the impact of dyspnea during activities of daily living acitvities (p = 0.001) and a improvement in the quality of life (p = 0.001). Conclusion: The program of pulmonary rehabilitation promoted a improvement in functional capacity, reducing the dyspnea and also in the quality of life.

O tabagismo é uma das principais causas da limitação ao fluxo aéreo. A DPOC é importante causa de morbi-mortalidade no mundo. A deterioração da função pulmonar associada aos efeitos sistêmicos conduz à intolerância ao exercício com piora do condicionamento físico, limitacão das atividades de vida diária e diminuição da qualidade de vida. Além disso, a dispneia também contribui para um pior prognóstico da doença. A reabilitação pulmonar com enfoque no exercício físico, como já descrito na literatura, tem sido considerada a alternativa mais efetiva para minimizar os efeitos do descondicionamento físico nesses pacientes. Objetivos: Avaliar o efeito de um programa de reabilitação pulmonar na capacidade funcional, na dispneia e na qualidade de vida em pacientes com DPOC. Metodologia: Trata-se de um ensaio clínico não controlado, aprovado pelo comitê de ética em pesquisa do HC-UFG. Foram avaliados 17 indivíduos com DPOC participantes do Ambulatório de Reabilitação Pulmonar da Clínica Clare no momento da admissão no programa e após 20 sessões de treinamento. Indivíduos com doenças que limitassem a execução do protocolo foram excluídos da pesquisa. As variáveis analisadas foram: idade, sexo, grau de obstrução pulmonar (VEF1), índice de massa corpórea (IMC), força muscular respiratória, índice de mortalidade BODE, qualidade de vida pelo Saint Georges Respiratory Questionnaire (SGRQ), a dispneia nas atividades de vida diária pela escala London Chest Activity of Daily Living (LCADL), índice de dispneia (MRC) e a capacidade funcional (TC6). Análise estatística: Para testar a normalidade dos dados foi utilizado o teste Kolgomorov-Smirnov; para a comparacão das variáveis pareadas antes e após a RP foi utilizado o teste t Student e para avaliar a associação entre as variáveis foi utilizada a análise de correlação de Pearson. Resultados: Participaram do estudo 13 pacientes, com média de idade de 72.46 ± 5.84 anos, VEF1 % prev. 48.54 + 11.14, IMC 23/89 + 2.80 Kg/m2. Houve melhora da capacidade funcional (p= 0.001), da força muscular inspiratória (p= 0.042), da dispneia pela escala MRC (p= 0.002), do impacto da dispneia nas AVDs (p= 0,001) e da qualidade de vida (p= 0.001). Conclusão: O programa de reabilitação pulmonar promoveu melhora da capacidade funcional, da dispneia e da qualidade de vida dos pacientes avaliados.Made available in DSpace on 2014-07-29T15:29:07Z (GMT).
