Knowledge and practices of the multidisciplinary team from primary health care about sarcopenia and nutrient drug interactions in elderly

Resumo: The objective was to describe the knowledge and practices of the multidisciplinary team of a Primary Health Care Unit (PHCU) regarding drug-nutrient interactions and sarcopenia in the elderly. This is an exploratory study of qualitative nature. Knowledge and practices were assessed through a semi-structured questionnaire. The interviewees did not know how to define sarcopenia, but they could reconcile the loss of muscle mass with its possible signs, symptoms, and impacts. The calf circumference assessment did not cover all the elderly in the community. No training or capacity building specifically addressed the health issues of the elderly. The interviewees believed in the existence of drug-nutrient interaction. The elderly had many difficulties in using medications correctly, and the most commonly used medications were hypoglycemic agents, antihypertensives, and anti-inflammatories. The population had many socioeconomic difficulties, and they needed food baskets. The eating habits of the elderly included a high consumption of carbohydrates and lipids, and a low consumption of proteins, vegetables, fruits, and greens. The bioavailability of nutrients, medications, and possible interactions must be taken into consideration so that nutritional status and drug treatment are not compromised. For, good nutritional intake allows the prevention and prevents the progression of sarcopenia. And Primary Health Care should act in the early diagnosis of sarcopenia and in the development of actions for the prevention, and management of sarcopenia and associated conditions.

Se trata de un estudio exploratorio de carácter cualitativo. Los conocimientos y las prácticas se evaluaron mediante un cuestionario semiestructurado. Los encuestados no pudieron definir la sarcopenia, pero pudieron conciliar la pérdida de masa muscular con sus posibles signos, síntomas e impactos. La evaluación de la circunferencia de la pantorrilla no cubrió a todos los ancianos de la comunidad. Ninguna formación o formación abordó específicamente los problemas de salud de las personas mayores. Los encuestados creían en la existencia de una interacción entre el fármaco y el nutriente. Los ancianos tenían muchas dificultades para utilizar correctamente los medicamentos y los más utilizados eran los hipoglucemiantes, antihipertensivos y antiinflamatorios. La población tenía muchas dificultades socioeconómicas y necesitaba canastas de alimentos. Los hábitos alimentarios de los ancianos incluían un alto consumo de carbohidratos y lípidos, y un bajo consumo de proteínas, verduras, frutas y verduras. Se debe tener en cuenta la biodisponibilidad de nutrientes, fármacos y las posibles interacciones, para que no se comprometa el estado nutricional y el tratamiento farmacológico. Bueno, una buena ingesta nutricional permite la prevención y previene la progresión de la sarcopenia. Y la Atención Primaria de Salud debe actuar en el diagnóstico precoz de la sarcopenia y en el desarrollo de acciones de prevención y manejo de la sarcopenia y patologías asociadas.O objetivo do presente estudo foi de analisar a abordagem das interações fármaco-nutriente e da sarcopenia nos idosos, por uma equipe de Atenção Primária à Saúde, bem como, descrever a sua importância. Trata-se de um estudo exploratório de caráter qualitativo, que foi avaliado através de um questionário semiestruturado e de referenciais teóricos relacionados ao tema. Em relação a sarcopenia, os entrevistados não souberam a sua definição, e não havia um protocolo estruturado e de ampla aplicabilidade para avaliação da massa muscular em idosos. Em relação as interações fármaco-nutriente, por mais que os entrevistados julgassem como verdadeira as interações, não souberam identificar quais medicamentos e quais nutrientes estariam correlacionados. Em relação aos hábitos alimentares, a população possuía muitas dificuldades socioeconômicas, ocasionando em um alto consumo de gêneros alimentícios ricos em carboidratos e lipídios, e um baixo consumo de proteínas, de frutas, legumes e verduras. No que tange a saúde do idosos, devemos considerar a biodisponibilidade dos nutrientes, os medicamentos e as possíveis interações, para que não haja um comprometimento do estado nutricional. Pois, o bom aporte nutricional permite a prevenção e impede a progressão da sarcopenia. Desse modo, devemos enfatizar a importância da atuação da equipe que compõe a Atenção Primária à Saúde no diagnóstico precoce, bem como em ações de prevenção e manejo das condições associadas ao envelhecimento, como a sarcopenia.   Palavras-chaves: Idoso.
