MALT lymphoma in rectum: complete regression after R-CHOP chemotherapy

Resumo: The mucosa-associated lymphoid tissue (MALT) lymphomas are B-cell non-Hodgkin that can arise in a number of extra nodal areas, being more frequent in the stomach. The primary presentation in the rectum is particularly rare, with few case reports. Because of its rarity, there is no consensus on the best treatment. Objective: To describe a case of MALT lymphoma in the rectum, in a female patient and their response to R-CHOP chemotherapy (rituximab, cyclophosphamide, doxorubicin, vincristine and prednisone). Methods: Descriptive study as a case report and literature review. Case report: A Woman with 55 yearsold began 4 years ago with hematochezia and tenesmus, associated with abdominal distension and feeling fullness. The patient developed asthenia and pain of weak intensity in the left upper quadrant. Had breast cancer history treated surgically and with chemotherapy for 7 years. After investigation, we performed the diagnosis of MALT lymphoma of the rectum, classified with stage II by Ann Arbor classification. Patient received 8 cycles of R-CHOP chemotherapy regimen and achieved complete remission. Conclusions: Given the rarity of presentation of MALT primary lymphoma of the rectum and the lack of consensus on the optimal treatment, we found that, through the present case, the chemotherapy treatment with R-CHOP protocol allows achieving complete remission effectively and safe.

A apresentação primária em reto é particularmente rara, havendo poucos relatos de casos. Por sua raridade, não há consenso sobre a melhor forma de tratamento. Objetivo: Descrever um caso de linfoma MALT em reto, em uma paciente do sexo feminino e a sua resposta ao tratamento quimioterápico R-CHOP (rituximab, ciclofosfamida, doxorrubicina, vincristina e prednisona). Métodos: Estudo descritivo na forma de relato de casos e revisão de literatura. Relato de caso: Mulher de 55 anos iniciou há 4 anos, quadro clínico de hematoquezia e tenesmo, associada a distensão e sensação de plenitude abdominal. Evoluiu com queixas de astenia e dor do tipo cólica de fraca intensidade em hipocôndrio esquerdo. Apresentava histó- rico de câncer de mama tratada cirurgicamente e com quimioterapia há 7 anos. Após investigação foi realizado o diagnóstico de linfoma MALT de reto, classificado com estádio II pela classificação de Ann Arbor. Paciente recebeu 8 ciclos de quimioterapia do regime R-CHOP e obteve remissão completa do tumor.
