Health Educational Practices of Exclusive Breastfeeding: A Study in a Neonatal ICU

Resumo: Introduction: Exclusive Breastfeeding (EBF) prevents more than six million deaths of children under one year of age annually and about two million deaths can be avoided if BF is exclusive for up to six months and practiced universally. Objective: To understand educational practices in Exclusive Breastfeeding Health in a Neonatal ICU. Method: Integrative review of queries in the databases: BVS, PubMed, Medline and Lilacs in the period 2012 - 2022. Descriptors were used individually or combined. N=809 publications were rescued, after exhaustive reading of titles and abstracts, n=332 published complete articles on the subject and available free of charge were selected. After applying the inclusion and exclusion criteria, n=120 studies were selected to be read in full. After these procedures, n=14 studies were selected with a focus on answering the guiding question, on the adequacy of the content used to support the structuring of this integrative review. Results: The performance of Health Education linked to Breastfeeding in the Neonatal ICU borders guidance on the management of breast milk, the performance of the milk bank in the ICU, the process of feeding premature infants with breast milk; promoting skin contact and breast sucking. Strengthens the transition from breastfeeding to breastfeeding and trains the support network and extra-hospital follow-up; prevention of the risks of abandoning EBF. It prepares the mother, the premature infant and their family for the hospital discharge process and provides guidance regarding the premature postpartum period; encourages the inclusion of parents, friends and family in the breastfeeding process; implements on the approach of pre-existing diseases, the impacts of COVID-19 and its risks related to mental disorders, as well as the development of the same for carrying out the AME. Conclusion: Health Education is an important tool for information, training, guidance, prevention and strengthening of bonds between mothers, babies, family members, health teams and institutions. It also articulates and transforms the entire support and follow-up network

Objetivo: Comprender las prácticas educativas en Salud de Lactancia Materna Exclusiva en una UTI Neonatal. Método: Revisión integradora de consultas en las bases de datos: BVS, PubMed, Medline y Lilacs en el período 2012 - 2022. Se utilizaron descriptores de forma individual o combinada. Se rescataron N=809 publicaciones, luego de la lectura exhaustiva de títulos y resúmenes, se seleccionaron n=332 artículos completos publicados sobre el tema y disponibles de forma gratuita. Después de aplicar los criterios de inclusión y exclusión, se seleccionaron n=120 estudios para ser leídos en su totalidad. Después de estos procedimientos, n=14 artículos fueron seleccionados con el objetivo de responder a la pregunta guía, en la adaptación del contenido utilizado para apoyar la estructuración de esta revisión integradora. Resultados: La actuación de la Educación en Salud vinculada a la Lactancia Materna en la UTI Neonatal bordea la orientación sobre el manejo de la leche materna, la actuación del banco de leche en la UTI, el proceso de alimentación del prematuro con leche materna; promoviendo el contacto con la piel y la succión de los senos. Fortalece la transición de la lactancia materna a la lactancia materna y capacita a la red de apoyo y seguimiento extrahospitalario; prevención de los riesgos de abandono de la LME. Prepara a la madre, al prematuro y a su familia para el proceso de alta hospitalaria y orienta sobre el puerperio prematuro; fomenta la inclusión de padres, amigos y familiares en el proceso de lactancia; implementa sobre el abordaje de enfermedades preexistentes, los impactos del COVID-19 y sus riesgos relacionados con los trastornos mentales, así como el desarrollo de los mismos para la realización de la AME. Conclusión: La Educación en Salud es una importante herramienta de información, formación, orientación, prevención y fortalecimiento de vínculos entre madres, bebés, familiares, equipos e instituciones de salud. También articula y transforma toda la red de apoyo y seguimiento.Introdução: O Aleitamento Materno Exclusivo (AME) previne mais de seis milhões de mortes de crianças com menos de um ano de idade anualmente e cerca de dois milhões de mortes podem ser evitadas se a AM for exclusiva até seis meses e praticada universalmente. Objetivo: Compreender as práticas educativas em Saúde de Aleitamento Materno Exclusivo em UTI Neonatal. Método: Revisão Integrativa de consultas nas bases de dados: BVS, PubMed, Medline e Lilacs no período 2012 - 2022. Os Descritores foram utilizados individualmente ou combinados. Foram resgatados n=809 publicações, após leitura exaustiva dos títulos e resumos, foram selecionadas n=332 artigos publicados completos sobre a temática e disponível gratuitamente. Após aplicação dos critérios de inclusão de exclusão, n=120 estudos foram selecionados para serem lidos na íntegra. Após esses procedimentos, selecionaram-se n=14 trabalhos com enfoque na resposta à questão norteadora, na adequação do conteúdo empregado para subsidiar a estruturação desta revisão integrativa. Resultados: A atuação da Educação em Saúde vinculada ao Aleitamento Materno na UTI Neonatal margeia a orientação sobre manejo do leite materno, a atuação do banco de leite na UTI, o processo de alimentação do prematuro com o leite materno; a promoção do contato com a pele e da sucção no seio. Fortalece a transição da amamentação para o seio e capacita a rede de apoio e acompanhamento extra-hospitalar; prevenção dos riscos do abandono do AME. Prepara a mãe, o prematuro e sua família para o processo de alta hospitalar e orienta quanto ao pós-parto prematuro; estimula a inclusão dos pais, amigos e familiares no processo da amamentação; implementa sobre a abordagem das doenças pré-existentes, dos impactos da COVID-19 e os seus riscos referentes aos transtornos mentais, bem como, o desenvolvimento das mesmas para realização do AME. Conclusão: a Educação em Saúde é uma importante ferramenta de informação, capacitação, orientação, prevenção e fortalecimento de vínculos entre as mães, bebês, familiares, equipes de saúde e instituições.
