Complications Associated with the Use of the Indwelling Catheter in Children and Adolescents

Resumo: | Introduction: Indwelling catheters are resources widely used in prolonged therapies due to the comfort and safety they provide. However, their usage does not exempt the occurrence of complications, including infectious processes. Objective: To describe the complications associated with the use of the indwelling catheters Port o cath type, in children and adolescents under cancer treatment or with hematological disease. Method: Documentary and descriptive research carried out in records of patients held in a specialized clinic and four hospitals in a city in the state of São Paulo, in the years from 2006 to 2010. The sample consisted of 61 medical records of children and adolescents aged 2 months to 17 years. Descriptive statistics of categorical variables and position measurements of continuous variables were carried out. Results: There was a 13.1% incidence of complications related to use of the Port o cath, and 6.6% for early complications, and same percentage for late complications associated with infectious processes. Three of the four catheters removed due to infection (75%) had been implanted in patients diagnosed with leukemia. The mean duration of catheter use was 506.3 days (23-1,335dias). Conclusion: The incidence of catheter withdrawals due to infection (6.6%) suggests that protocols for manipulation of catheters and investment in improving staff capacity and measures aimed at reducing complications need to be adopted. |

Introducción: Los catéteres de larga duración son recursos ampliamente utilizados en terapias prolongadas debido al conforto y seguridad que promueven. Sin embargo, su uso no exime a la aparición de complicaciones, tales como procesos infecciosos. Objetivo: Describir las complicaciones asociadas con el uso de un catéter totalmente implantable del tipo Port o cath, en niños y adolescentes en tratamiento contra el cáncer o enfermedad hematológica. Método: Investigación documental y descriptiva realizada en la historia clínica de los pacientes de una consulta especializada y en cuatro hospitales en una ciudad del interior de São Paulo, entre los años 2006 y 2010. La muestra fue constituida por 61 historias clínicas de niños y adolescentes entre 2 meses y 17 años de edad. Se realizó una estadística descriptiva de las variables categóricas y las mediciones de posición de las variables continuas. Resultados: Se observó una incidencia del 13,1% de complicaciones relacionadas con el uso del Port o cath, y el 6,6% de complicaciones precoces, e igual porcentaje para las complicaciones tardías asociadas a procesos infecciosos. Tres de los cuatro catéteres retirados debido a la infección (75%) habían sido implantados en los pacientes diagnosticados con leucemia. El tiempo promedio de permanencia del catéter fue de 506,3 días (23-1.335días). Conclusión: La incidencia de retiradas del catéter debido a infecciones (6,6%) sugiere la necesidad de adoptar protocolos para la manipulación de catéteres y la inversión em capacitación del personal con la finalidad de reducir las complicaciones. Introdução: Os cateteres de longa permanência são recursos amplamente utilizados em terapias prolongadas devido ao conforto e à segurança que promovem. Entretanto, seu uso não isenta a ocorrência de complicações, entre as quais os processos infecciosos. Objetivo: Descrever as complicações associadas ao uso do cateter totalmente implantável do tipo Port o cath, em crianças e adolescentes em tratamento oncológico ou de doença hematológica. Método: Pesquisa documental e descritiva, realizada em prontuários de pacientes de um ambulatório especializado e em quatro hospitais de uma cidade do interior Paulista, nos anos de 2006 a 2010. A amostra foi constituída por 61 prontuários de crianças e adolescentes com idade entre 2 meses a 17 anos de idade. Realizada estatística descritiva das variáveis categóricas e medidas de posição das variáveis contínuas. Resultados: Verificou-se incidência de 13,1% de complicações relacionadas ao uso do Port o cath, sendo 6,6% para complicações precoces, e igual percentual para complicações tardias associadas a processos infecciosos. Três dos quatro cateteres retirados por infecção (75%) haviam sido implantados em pacientes com diagnóstico de leucemia. O tempo médio de permanência do cateter foi de 506,3 dias (23-1.335dias). Conclusão: A incidência de retiradas do cateter por infecção (6,6%) sugere a necessidade de adoção de protocolos para manipulação dos cateteres e investimentos em capacitação de pessoal como medidas voltadas à redução das complicações.
