THE CONVENIENCE OF CLINICAL PLUS NUTRmONAL INTERVENTIONS IN GASTROENTEROLOGICAL PATIENTS: CASE REPORT

Resumo: | The advantages of the multiprofessional approach for diagnosis and treatment of hospitalized patients is becoming evident and mandatory. The result of this multiprofessional interaction is analysed in the present article under the view of nutritional therapy. Two mala adults patients suffering from chronic diarrhea of two distinct etiologias: traumatic retite and lymphoproliferative disease were both severely malnourished at the admittance. The nutritional assessment (dietary intake, anthropometry and blood biochemistry) was done weekly throughout the hospitalization. Dietary intervention consisted of either oral (obstipant), enteral and/or parenteral supports. Both patients reacted well to diets decreasing the defecation rate and doubling their serum leveis of free tryptophan. However the nutritional improvement was sustained only in the case where the basic disease (lymphoproliferative disease) was conveniently treated. ln the other case the anal-sphincter reconslruclion was not undertaken, and probably because of that lhe improvemenl of nutritional indicalors was transienl and ceased soon after the dielary support. So it was evidenl the inability of diel therapy in keeping atone the nutritional state of the patienl without lhe adequale clinical-surgical procedure. |

A necessidade da abordagem multiprofissional da terapêutica dos pacientes internados é cada dia mais evidente e imperiosa. O resultado dessa integração é analisado, no presente trabalho, sob a óptica da terapêutica nutricional. Dois pacientes adultos, portadores de diarréia crônica de etiologias diferentes (retite traumática e doença linfoproliferativa) ambos com desnutrição protéico-calórica, avaliada mediante ingestão alimentar, antropometria e exame bioquímico, foram submetidos à dietoterapia oral (obstipante), enteral e/ou parenteral. A resposta nutricional satisfatória de ambos os pacientes à dietoterapia pode ser detectada pela elevação (de 2 a 3 vezes) dos níveis séricos de triptofano livre e redução de 10 a 12 vezes na freqüência das evacuações. Entretanto, a melhora nutricional foi mantida apenas no caso em que a doença de base (doença linfoproliferativa) sofreu tratamento clínico adequado. Essa melhora foi apenas transitória no outro caso, em que a conduta cirúrgica (reconstrução do esfíncter anal) não foi efetivada.
