Resumo: | Introduction: Bronchiolitis obliterans post infecction (PIBO) is a chronic obstructive pulmonary disease characterized by obliteration of the distal airways caused by fibrosis and scarring of lung tissues. Assessment of pulmonary function, and particularly the demonstration of reversibility of obstruction, may assist in the diagnosis and evaluation of respiratory system impairment. The forced oscillation technique (FOT) is a functional evaluation method with a high success rate in situations of poor patient collaboration and describes mechanical properties of the respiratory system that are not evidenced in spirometry. Objective: To evaluate the accuracy of spectral FOT and within-breath for the detection of BO. Methods: Cross-sectional study. The sample was selected for convenience, being composed of children and adolescents with BO and controls, aged between 4 and 19 years. Anthropometry, clinical history and measures of respiratory impedance were obtained in all participants. Spirometry was performed in the BO group. Results: 16 subjects with BO and 32 healthy individuals were recruited. Patients with BP had a more negative response (p=0.001), with more negative inertance (p=0.001) and a higher frequency of resonance (p=0.029) when compared to patients with BP (X6hz, 8hz, 10hz, p <0.05) controls. The variables of the intra-breath analysis with greater sensitivity to differentiate the BO subjects of the controls were XmédiaE, XmédiaI and XeE. Conclusions: The results showed that the individuals with BO present significantly more negative reactance in the low frequencies by the within-breath method, with consequent elevation of fRes. |

A avaliação da função pulmonar, e particularmente a demonstração da reversibilidade da obstrução pode auxiliar no diagnóstico e avaliação do comprometimento do sistema respiratório. A técnica de oscilações forçadas (FOT) é um método de avaliação funcional com alto índice de sucesso em situações de pouca colaboração por parte do paciente e descreve propriedades mecânicas do sistema respiratório que não são evidenciadas na espirometria. Objetivo: Descrever a função pulmonar de portadores de BOPI pela FOT. Métodos: Estudo transversal. A amostra foi selecionada por conveniência, composta por crianças e adolescentes com BOPI e controles saudáveis, com idade entre 4 e 19 anos. Antropometria, história clínica e medidas da impedância respiratória foram obtidos em todos os participantes. Espirometria foi realizada no grupo BOPI. Resultados: Foram recrutados 16 indivíduos com BOPI (15 realizaram função pulmonar) e 32 indivíduos sadios (29 realizaram função pulmonar). Na análise espectral os pacientes com BOPI apresentaram reatância pulmonar (X6hz,8hz,10hz, p<0,05), complacência (p=0,008) e inertância (p=0,001) mais negativas e frequência de ressonância (p=0,029) mais elevada quando comparados aos controles. Na análise com modelo within-breath detectamos diferenças significativas de R e X nas variáveis de eE, maxI, eI e maxE (p<0,05) quando comparados aos controles. As variáveis com maior sensibilidade para diferenciar os sujeitos BOPI dos controles foram XeE, XeI e XmédiaI (ROC, AU: 0,80, 0,82 e 0,80 respectivamente). As variáveis de reatância XeE, XeI, XmaxI e XmaxE apresentam correlação significativa com FEF25-75% (p<0,05). Conclusões: Este é o primeiro estudo realizado que utiliza a FOT em BOPI no Brasil. Os resultados demonstraram que os indivíduos portadores de BOPI apresentam reatância significativamente mais negativa nas frequências baixas o que sugere hiperinsuflação pulmonar, redução na complacência do sistema respiratório e heterogeneidade da ventilação. As medidas de resistência também foram estatisticamente significativas, mas de menor magnitude.
