Estudo comparativo entre a oclusão de modelos de gesso e oclusão virtual em planejamentos de cirurgia ortognática

Resumo: | Orthognathic surgery is the procedure used for the correction of dentofacial deformities. The success of the procedure depends on both surgical skill and accurate and detailed diagnosis and planning. The diagnosis and, especially, the planning of those procedures were highly influenced in the last years by the technological development. The development of software and hardware helped the emergence of diagnostic tools and threedimensional planning, making these surgeries more precise and accurate. However, there are still many doubts about the fidelity of the planning or occlusal diagnosis totally virtual, without the presence of physical models in plaster, for example. This scientific work seeks to study occlusal stability through dental contact in virtual planning software and will be presented in two scientific articles. Dolphin Imaging software 11.9v. was used for the surgical planning of surgeries with the use of two sets of occlusal models. One with the occlusion obtained in a totally virtual way and the other with the final occlusion obtained through physical models, scanned in position of virtual occlusion. Article 1 presents the validation of the study model of occlusal stability measured in both occlusions. Article 2 presents the results of the proposed methodology through the contact area in pixels. The results allow to conclude that the best fit surface obtained through totally virtual resources presents differences in the final position of the occlusion obtained 10 through the manipulation of physical models. However, the results were not statically significant between the occlusal contact areas obtained, although the surface obtained and measured by the virtual method was higher. More tools for occlusal virtual planning have yet to be developed and further studies must be conducted so that the results obtained encourage more surgeons to perform surgical planning in a completely virtual manner without the need for laboratorial steps in the process. |

O sucesso do procedimento depende tanto da habilidade cirúrgica quanto de um diagnóstico e planejamento preciso e detalhado. O diagnóstico e, principalmente, o planejamento desses procedimentos foram altamente influenciados nos últimos anos pelo desenvolvimento tecnológico. O desenvolvimento de softwares e hardwares auxiliaram o surgimento de ferramentas diagnósticas e de planejamento tridimensional, tornando essas cirurgias mais precisas e acuradas. No entanto, ainda existem muitas dúvidas a respeito da fidelidade do planejamento ou diagnóstico oclusal totalmente de maneira virtual, sem a presença de modelos físicos em gesso, por exemplo. Este trabalho científico busca estudar a estabilidade oclusal por meio das áreas de contatos dentários em software de planejamento virtual, sendo apresentado em dois artigos científicos. O software Dolphin Imaging 11.9v. foi utilizado para o planejamento cirúrgico dos procedimentos com a utilização de dois conjuntos de modelos oclusais. Um com a oclusão obtida de forma totalmente virtual e o outro com a oclusão final obtida através de modelos físicos, escaneados em posição de oclusão virtual. O artigo 1 apresenta a validação do modelo de estudo por meio da verificação e comparação do posicionamento dos modelos em ambos os métodos. Já o artigo 2 apresenta os resultados da estabilidade oclusal medida por meio da comparação da área de contato obtida em pixels. Os resultados permitem concluir que a oclusão obtida mediante recursos totalmente virtuais apresenta diferenças de posicionamento em relação à oclusão obtida mediante manipulação de modelos físicos. No entanto, não se encontrou diferenças significativas entre as áreas de contato oclusal obtidas, apesar da superfície obtida e mensurada pelo método virtual ter sido maior.
