Resumo: Cognitive-motor changes have an important contribution to mobility impairment in the elderly, which is usually followed by an increasing risk of falls. Oldest old are elderly people over 80 years of age, who, unfortunately, are more vulnerable to such events. In this context, the act of turning, due to its biomechanical characteristics, is known to increase susceptibility to postural instability. Although gait stability has been widely studied in the elderly, research on changes in nonlinear gait features among the oldest old is scarce. Moreover, regarding the history of fall, the predictive value of cognitive-motor performance while walking in the home environment in the oldest old is unknown. Therefore, the present study aims to evaluate the predictive capacity of the simple Timed Up and Go (TUG) and cognitive-motor TUG tests for the history of falls in the non-institutionalized oldest old. In this case-control study, the tests were carried out in the participants? residences. Six valid attempts of the TUG test were performed, 3 in simple mode and 3 in DT, using the inertial sensor G-Walk and a smartphone Moto G5 to record the tests. The sample consisted of 60 oldest old subjects aged ?85, from Porto Alegre, Rio Grande do Sul. Socio-demographic data, levels of physical activity, mental state, presence of depressive symptoms, concern about the occurrence of falls, medicine use, self-perception of functional balance and the functional scope test were also assessed using specific instruments. Descriptive statistics were used to characterize the sample. The TUG modalities (simple and TD) performance was compared using repeated measures ANOVA. Finally, the prediction of history of falls was assessed using binary logistic regression. All analyzes were performed in the SPSS 17.0 statistical package. The results showed that the elderly in this study, in general, were physically active and had no apparent changes in mental status. Whilst DT reduced the performance of the oldest old during the test, this TUG modality did not provide a greater predictive capacity to distinguish between fallers and non-fallers. However, the non-linear gait (turning 180?) was superior to the total TUG time for predicting the chance of falls, in both single and DT task. Finally, being physically active was the main independent protective factor to the occurrence of falls.

Neste contexto, sabemos que o ato de girar, aumenta a susceptibilidade ? instabilidade postural, haja vista a natureza de suas caracter?sticas biomec?nicas. Apesar da estabilidade da marcha ser bastante estudada em idosos, at? o presente momento, ainda n?o se conhece a contento as altera??es da marcha n?o-linear na popula??o de idosos longevos, tampouco se o desempenho cognitivo-motor durante tal situa??o pode ser um bom preditor independente do hist?rico de quedas em longevos avaliados em seu ambiente domiciliar. Al?m disso, ainda n?o se sabe se a avalia??o da marcha em dupla-tarefa cognitivomotora (DT) pode agregar valor para a determina??o da chance de quedas em longevos. Portanto, a presente pesquisa teve como objetivo principal avaliar a capacidade de predi??o dos testes Timed Up and Go (TUG) simples e em DT cognitivo-motora para o hist?rico de quedas em longevos n?o-institucionalizados. Neste estudo caso-controle, as avalia??es ocorreram nas resid?ncias dos longevos, onde realizou-se seis tentativas v?lidas do teste TUG, sendo 3 em modo simples e 3 em DT, com o aux?lio do sensor inercial G-Walk e um smartphone Motorola para filmagem dos testes. A amostra foi composta por 60 longevos com idade ?85 anos, residentes na cidade de Porto Alegre, Rio Grande do Sul. Dados sociodemogr?ficos, n?veis de atividade f?sica, estado mental, presen?a de sintomas depressivos, preocupa??o com a ocorr?ncia de quedas, f?rmacos em uso, autopercep??o de equil?brio funcional e o teste do alcance funcional tamb?m foram avaliados por instrumentos espec?ficos. A caracteriza??o da amostra deu-se por meio da estat?stica descritiva. A compara??o de desempenho entre as modalidades do TUG (simples e DT) foi realizada pela ANOVA de medidas repetidas. Por fim, a predi??o da chance de quedas foi realizada com o emprego da regress?o log?stica bin?ria. Todas as an?lises foram realizadas no pacote estat?stico SPSS 17.0. Os resultados demonstram que os idosos inclu?dos, eram fisicamente ativos e n?o possu?am altera??es aparentes quanto ao estado mental. A realiza??o do TUG em DT, embora reduza o desempenho dos idosos durante a realiza??o do teste, n?o confere uma maior capacidade preditiva ao teste para diferenciar sujeitos caidores e n?o caidores. No entanto, a fase de giro 1800 ou seja a transi??o entre a marcha de ida e a de volta, mostrou-se superior ao tempo total do TUG para a predi??o da chance de quedas, tanto em tarefa simples quanto em DT.
