Lesão Iatrogénica Biliar - Resultados de Inquérito Nacional 2005-2007

Resumo: The number and severity of bile duct injuries has increased since the introduction of laparoscopic cholecystectomy. This study determines their incidence, clinical presentation and management in the setting of Portuguese public hospitals, by means of a national inquiry promoted by the HPB chapter of the Portuguese Surgical Society. A 37% response rate was obtained from 29 hospitals having performed collectively 15533 cholecystectomies during a 3 year period. Out of 88 bile duct lesions, calculating an incidence of 0,57%, 10 were minor, due to leakage from the cystic duct. No biliary risk factors were present in 57% of the affected patients. Thirty lesions were recognized intra-operatively (35%), 34 during the first week, and 24 more than a week later. The most common presentation was a fistula (58%). Primary treatment was surgical in 71 patients, endoscopic in 9 and percutaneous (interventional radiology) in 8. Five patients died (5,7%). Almost 2 decades after the first laparoscopic cholecystectomy in Portugal the rate of bile duct injury is still high, treatment remains essentially surgical and fails in approximately one third of cases followed-up for 2,5 years. Keywords: Cholecystectomy, laparoscopy, iatrogenic, treatment As lesões das vias biliares, cuja incidência aumentou com a introdução da colecistectomia laparoscópica, são a mais séria das complicações deste procedimento. Este estudo, promovido pelo capítulo de Cirurgia HPB da Sociedade Portuguesa de Cirurgia, caracteriza a sua actual incidência, formas de apresentação e tratamento, em hospitais públicos portugueses, através de um Inquérito Nacional. Responderam ao inquérito 29 hospitais (37%), com um total de 15533 colecistectomias, num período de 3 anos. Declararam-se um total de 88 lesões biliares, correspondendo a uma incidência global de 0,57%. Destas, 10 eram minor, com ponto de partida no canal cístico. Não havia factores de risco vesiculares em 57% dos doentes. Trinta lesões foram reconhecidas intra-operatoriamente (35%), das quais 27 foram reparadas imediatamente, 34 na primeira semana (38%), 24 depois da primeira semana (27%). A manifestação clínica mais frequente foi a fístula biliar (58%). O tratamento inicial foi cirúrgico em 71 doentes, endoscópico em 9, ou intervenção percutânea em 8. A mortalidade global foi de 5 doentes (5,7 %). Quase 2 décadas após a sua introdução em Portugal a incidência de lesão iatrogénica continua elevada (0,57%), e, sendo o seu tratamento inicial cirúrgico, mantém uma taxa de insucesso (33%) igualmente elevada.
