The contribution of the diffusion-weitghted imaging in breast carcinoma diagnosis

Resumo: | Objective: To investigate the mean apparent diffusion coefficient values of different tissues and their real capability in distinguish the different types of histological tissues of breast lesions. Materials and Methods: A descriptive and correlational research has been developed in which apparent diffusion coefficient values of breast MRI examinations, performed in a public radiology service, has been collected. The sample included 31 patients, between 32 and 75 years old, with breast lesions confirmed by biopsy or surgical specimen. The treatment and statistical analysis were carried out using Excel and SPSS. Results: Benign lesions had mean apparent diffusion coefficient values of 1.58 x10-3mm2/s, while malignant lesions had mean apparent diffusion coefficient values of 0.89 x10-3mm2/s. Regarding to the various types of histological tissues, we found mean apparent diffusion coefficient values for invasive ductal carcinoma of 0.87x10-3mm2/s, 0.99x10-3mm2/s for ductal carcinoma in situ, and 0.85x10- 3 mm2/s for invasive lobular carcinoma. Conclusion: The results demonstrate that the apparent diffusion coefficient values enable to differentiate between benign lesions, malignant lesions and healthy fibroglandular tissue, but it was not possible to set ranges of apparent diffusion coefficient values between the different histological types of lesions. We could only identify ranges of apparent diffusion coefficient values between malignant lesions and healthy fibroglandular tissue. |

Materiais e Métodos: Desenvolveu-se um estudo descritivo-correlacional, no qual se realizou a recolha dos valores de coeficientes aparentes de difusão referentes aos exames de ressonância magnética mamária realizados durante um semestre num serviço público de radiologia. A amostra incluiu 31 utentes, entre os 32 e os 75 anos, com lesão mamária confirmada através de biópsia ou peça operatória. O tratamento e análise estatística foram efetuados com recurso ao Excel e SPSS. Resultados: As lesões benignas apresentaram valores médios de coeficientes aparentes de difusão de 1,58 x10-3mm2/s, enquanto as lesões malignas apresentaram valores coeficientes aparentes de difusão médios de 0,89 x10-3mm2/s. Relativamente aos vários tipos de tecidos histológicos encontrámos valores de coeficientes aparentes de difusão médios para o carcinoma ductal invasivo de 0,87 x10-3mm2/s, de 0,99 x10-3mm2/s para o carcinoma ductal in situs e de 0,85x10-3 mm2/s para o carcinoma lobular invasivo.
