Treatment of the pilonidal disease: experience with the Karydakis flap

Resumo: The classic methods for the treatment of pilonidal disease have been questioned by the discomfort and degree of inability they impose on patients. Primary closure with a flap procedure allows for a reduction of healing time, post operative pain, inability and recurrence rate. Of all flap procedures, the one described by Karydakis is one of least technically demanding and with a shorter learning curve. A retrospective study was elaborated based on hospital records for all patients with chronic pilonidal disease, who underwent surgical treatment with the Karydakis flap in the General Surgery Department I – Centro Hospitalar de Lisboa Ocidental, between February 2004 and June 2007, 99 patients with chronic pilonidal disease were operated upon. It was possible to obtain from the revision of the clinical records: a recurrence rate of 6%, a mean healing time of 17 days and a minor wound complication rate of 14%. These results are in accordance with the published literature, validating this technique in our institution.

These results are in accordance with the published literature, validating this technique in our institution. Os métodos clássicos de tratamento da doença pilonidal (exerése com encerramento secundário ou com encerramento primário na linha mediana) têm vindo a ser questionados pelo tempo de incapacidade e pelo desconforto que impõem aos doentes. O encerramento primário com plastia permite reduzir o tempo de cicatrização, a dor no pós-operatório, o tempo de incapacidade e a taxa de recidiva. De todas as técnicas com plastia, aquela descrita por Karydakis é seguramente uma das de mais fácil execução e menor curva de aprendizagem. Procedeu-se ao estudo retrospectivo dos registos hospitalares de todos os doentes operados a sinus pinolidalis em fase de cronicidade, pela técnica de Karydakis, no período que decorreu entre Fevereiro de 2004 e Junho de 2007, no Serviço de Cirurgia I – Centro Hospitalar de Lisboa Ocidental (CHLO). Neste período, foram operados 99 doentes com doença pilonidal em fase de cronicidade. Da revisão dos registos operatórios obteve-se uma taxa de recidiva de 6%, um tempo médio de cicatrização de 17 dias e uma taxa de complicações da ferida operatória de 14%. Estes resultados estão de acordo com a literatura publicada, validando a utilização desta técnica na nossa instituição.
