Ajustamento Di?dico em Casais com Infertilidade a Realizar Tratamento M?dico e Candidatos a Ado??o

Resumo: Introdu??o: A infertilidade ? uma condi??o que causa impacto a n?vel pessoal, social, financeiro e conjugal. A maioria dos casais submete-se a tratamentos m?dicos, no entanto, ou porque estes n?o t?m sucesso, ou por outras raz?es, t?m tamb?m a op??o de adotar. Independentemente da escolha do casal para a concretiza??o do seu desejo parental, bons n?veis de ajustamento di?dico s?o importantes para lidar com os desafios inerentes ? escolha. Objetivos: Averiguar a exist?ncia de diferen?as entre um grupo de casais com infertilidade a realizar tratamento (GI) e um grupo de casais candidatos ? ado??o (GA) no que se refere a vari?veis demogr?ficas, cl?nicas, de sa?de mental, ajustamento di?dico e regula??o emocional. Adicionalmente, procurou-se explorar quais destas vari?veis atuavam como preditores do ajustamento conjugal. Metodologia: O recrutamento dos participantes foi realizado atrav?s da Associa??o Portuguesa de Fertilidade, e do Instituto de Seguran?a Social. Participaram 67 casais no GI e 59 casais no GA. Todos eles preencheram um question?rio sociodemogr?fico e um conjunto de instrumentos de autorresposta, mais precisamente a Positive and Negative Affect Schedule, a Others as Shamer, a Self-Compassion Scale e a Dyadic Ajustment Scale. O papel preditor das vari?veis no ajustamento di?dico dos dois grupos foi analisado atrav?s de regress?es lineares m?ltiplas. Resultados: Os casais do GI, quando comparados com os casais do GA, apresentaram n?veis mais elevados de afeto negativo e autojulgamento e n?veis mais baixos de afeto positivo, de autocompaix?o e de ajustamento di?dico. No GI a vergonha externa foi o ?nico preditor significativo do ajustamento di?dico, enquanto no GA foram o afeto positivo e negativo os preditores desta vari?vel. Discuss?o: Atendendo a que o ajustamento di?dico se constitui como um elemento importante nos casais que enfrentam um diagn?stico de infertilidade e as exig?ncias do tratamento m?dico ou de um processo de candidatura a ado??o, o conhecimento dos seus preditores ? relevante. Assim, interven??es que visem a diminui??o dos n?veis de vergonha em casais a realizar tratamento m?dico de infertilidade e abordem os n?veis de afeto em casais candidatos a ado??o, podem potenciar o ajustamento di?dico. / Introduction: Infertility is a medical condition with impact on a personal, social, financial and marital level. Most couples undergo medical treatments, however, either because these are not successful or for any other reason, they also have the option to pursue adoption. Regardless of the couple's choice to fulfill their wish for a child, good levels of dyadic adjustment are important to deal with their choice inherent challenges. Objectives: To investigate the existence of differences between a group of couples with infertility undergoing treatment (GI) and a group of couples pursuing adoption (GA) regarding demographic, clinical, mental health, dyadic adjustment and emotion regulation. Additionally, the predictive role of these variables on marital adjustment was explored. Methodology: Participants were recruited through the Associa??o Portuguesa de Fertilidade and the Instituto de Seguran?a Social. Sixty-seven couples participated in the GI and 59 couples in the GA. All of them completed a sociodemographic questionnaire and a set of self-report instruments, more precisely the Positive and Negative Affect Schedule, the Others as Shamer, the Self-Compassion Scale and the Dyadic Adjustment Scale. The predictive role of these variables on the dyadic adjustment of the two groups was analyzed using multiple linear regression analyses. Results: The GI couples, compared to the GA couples, showed higher levels of negative affect and self-judgment and lower levels of positive affect, self-compassion and dyadic adjustment. In the GI, external shame was the only significant predictor of dyadic adjustment, while in the GA, positive and negative affect were the predictors of this variable. Discussion: Dyadic adjustment is an important component for couples facing an infertility diagnosis and the demands of medical treatment or an adoption application. Therefore, knowing dyadic adjustment predictors may be relevant. Interventions aimed at reducing shame levels in couples undergoing medical treatment for infertility and addressing the positive and negative affect levels in couples applying for adoption can enhance dyadic adjustment.
