Material e Métodos: Este estudo observacional retrospetivo, de dados colhidos prospetivamente, estudou mulheres que realizaram cirurgia com slings da uretra média para o tratamento da incontinência urinária de esforço. As consultas de seguimento foram marcadas para 6, 12, 24 e 48 meses pós-cirurgia. Um teste da tosse com resultado negativo foi usado como definição de cura objetiva. Para avaliar os outcomes subjetivos foi usado o King’s Health Questionnaire. As complicações pós-operatórias também foram registadas. Resultados: Das 207 mulheres que foram analisadas, 63 foram tratadas com Ajust™ e 144 com SMUS. Não houve diferenças estatisticamente significativas entre os dois grupos tanto em termos de cura objetiva como cura subjetiva, assim como em termos de melhoria subjetiva, em todas as etapas temporais das consultas de seguimento. Na consulta de seguimento dos 48 meses pós-operatórios, a taxa de cura objetiva foi 93.8% no grupo da Ajust™ contra 94.2% no grupo SMUS (p = 1.000) e a taxa de cura subjetiva foi 77.1% no grupo Ajust™ contra 72.5% no grupo SMUS (p = 0.604). Nessa mesma etapa temporal, 14.6% do grupo Ajust™ contra 9.8% do grupo SMUS não atingiram a cura subjetiva, mas obtiveram pelo menos uma melhoria clinicamente significativa na qualidade de vida após a cirurgia (p = 0.466). Não foram detetadas diferenças significativas no que diz respeito às complicações pós-operatórias. Conclusão: O sling de incisão única Ajust™ aparenta ser tão eficaz como os SMUS em termos de outcomes objetivos e subjetivos a longo prazo. Além disso, ambos os métodos cirúrgicos parecem ser igualmente seguros para o tratamento da incontinência urinária de esforço.Objective: The aim of this study was to compare the long-term objective and subjective cure rates of the adjustable single incision sling Ajust™ versus standard midurethral slings (SMUS) in the surgical treatment of female stress urinary incontinence. Material and Methods: This was a retrospective observational study of prospectively collected data regarding women who underwent midurethral sling surgery to treat stress urinary incontinence. Follow-up was scheduled at 6, 12, 24, and 48 months following surgery. A negative cough stress test was used as the definition of objective cure. Subjective outcomes were assessed with the King’s Health Questionnaire. Postoperative complications were also recorded. Results: Of the 207 women who were analysed, 63 were treated with Ajust™ and 144 with SMUS. There were no statistically significant differences between the two groups in terms of both objective and subjective cure, as well as subjective improvement, at each and every follow-up time point. At the 48-month control, the objective cure rate was 93.8% in the Ajust™ group vs 94.2% in the SMUS group (p = 1.000), and the subjective cure rate was 77.1% in the Ajust™ group vs 72.5% in the SMUS group (p = 0.604). At that same time point, 14.6% in the Ajust™ group and 9.8% in the SMUS group did not achieve subjective cure, but had at least a clinically significant improvement in their quality of life following incontinence surgery (p = 0.466). No significant differences regarding postoperative complications were noted. Conclusion: The Ajust™ single incision sling procedure appears to be just as effective as SMUS in terms of objective and subjective long-term outcomes.
