REMOTE NURSING CARE TO THE FAMILY OF THE CRITICAL PATIENT WITH COVID-19: PIONEERING TECHNOLOGY IN TERESINA-PI: Cicatrização de Lesão em Neuropatia Diabética: Relato de Caso

Resumo: ABSTRACT Objective: To describe the impact of treatment with papain, high frequency and low intensity laser on the healing of an acute complex skin lesion due to diabetic peripheral vascular neuropathy. Method: This is a case report study, carried out in a Multidisciplinary Complex Skin Injury Outpatient Clinic. The study covered 19 weeks and used the Pressure Ulcer Scale for Healing (PUSH) and photographic records. Results: Throughout the treatment, the 10 cm² lesion, initially exudative and with devitalized tissue, showed notable improvement in the fifth week. The wound remained stable between the seventh and 13th week, followed by a significant decrease in extension, culminating in complete healing at week 19. The PUSH scale corroborated this evolution, consistently reflecting a reduction in scores. Images documented the progress of the wound, highlighting complete healing at week 19. The varied prescription of papain and control of hyperglycemia contributed to the success of the treatment, with the patient consistently adhering to follow-up. Final considerations: The study highlighted that low-cost therapies can promote the healing of injuries in diabetic patients with peripheral neuropathy, improving quality of life and avoiding amputations. Despite limitations, the results indicate a positive evolution in the healing of the lesion over the 19 weeks of treatment, with a reduction in the area, a reduction in devitalized tissue and complete epithelialization, validated by the PUSH scale. Método: Se trata de un estudio de reporte de caso, realizado en un Ambulatorio Multidisciplinario de Lesiones Cutáneas Complejas. El estudio duró 19 semanas y utilizó la Escala de curación de úlceras por presión (PUSH) y registros fotográficos. Resultados: A lo largo del tratamiento la lesión de 10 cm², inicialmente exudativa y con tejido desvitalizado, presentó notable mejoría en la quinta semana. La herida permaneció estable entre la séptima y la decimotercera semana, seguida de una disminución significativa en la extensión, que culminó con la curación completa en la semana 19. La escala PUSH corroboró esta evolución, reflejando consistentemente una reducción en las puntuaciones. Las imágenes documentaron el progreso de la herida, destacando la curación completa en la semana 19. La variada prescripción de papaína y el control de la hiperglucemia contribuyeron al éxito del tratamiento, observándose el seguimiento constante del paciente. Consideraciones finales: El estudio destacó que terapias de bajo costo pueden promover la curación de lesiones en pacientes diabéticos con neuropatía periférica, mejorando la calidad de vida y evitando amputaciones.
