Avaliação do uso de bactérias probióticas na pancreatite aguda induzida em ratos

Resumo: | Acute pancreatitis presents high rates of morbidity and mortality, mainly when it is associated with infection of pancreatic necrosis. The mechanism of this occurrence has not been fully elucidated, but may possibly be due to the bacterial translocation caused by disruption of the intestinal barrier. The objective of the present study was to determine the effect of dietary supplementation with probiotic bacteria (PB) on acute pancreatitis (AP) induced in rats by the inoculation of 5% sodium taurocholate into the biliopancreatic duct. Fifty Sprague-Dawley rats were randomly divided into five groups: 1 sham-operated, with manipulation of intra-abdominal organs; 2 induction of AP, without supplementation with PB; 3 dietary supplementation with PB followed for induction of AP; 4 induction of AP followed by dietary supplementation with PB; and 5 dietary supplementation with PB before and after the induction of AP. 4 x 109 colony-forming units (CFU) of probiotic bacteria were directly administered into the stomach, divided into two stages per day. Blood samples were collected for the determination of serum levels of amylase, lipase, glucose, and calcium and for leukocyte counts immediately before (0 hour) and 24 and 48 hours after the surgical procedure. The animals were sacrificed and ileal lymph nodes and fragments of pancreas, spleen, and liver were obtained. Comparison of the five groups of animals revealed significant differences in leukocyte count at 24 hours and in amylasemia and lipasemia at 24 and 48 hours after AP induction. Considering only the groups with AP induction (groups 2, 3, 4, and 5), there was no difference in any of the parameters assessed or in bacterial colonization of the organs. When considering only the groups of animals that received PB (groups 3, 4, and 5), a greater occurrence of lymph node contamination and of pancreatic hemorrhage was observed compared to the other groups. The present results suggest that dietetary supplementation with this combination of PB in animals submitted to the induction of AP does not modify the profile of the biochemical markers evaluated or the leukocyte count, but increases the rate of bacterial translocation to lymph nodes and the occurrence of pancreatic hemorrhage. |

O mecanismo exato para esta ocorrência ainda não foi totalmente elucidado, mas, possivelmente, é oriunda da translocação bacteriana decorrente da disfunção da barreira intestinal. O objetivo deste estudo foi avaliar o efeito da suplementação dietética com bactérias probióticas (BP) na resposta a pancreatite aguda (PA) induzida em ratos pela inoculação de taurocolato de sódio a 5% no ducto biliopancreático. Cinqüenta ratos Sprague-Dawley foram aleatoriamente distribuídos em cinco grupos: 1 operação simulada, com manipulação de órgãos intra-abdominais; 2 indução de PA, sem suplementação de BP; 3 suplementação dietética com BP, seguida por indução de PA; 4 indução de PA, seguida por suplementação dietética com BP; e 5 suplementação dietética com BP, antes e após a indução de PA. A suplementação de BP se deu pela administração por via intragástrica de 4 x 109 unidades formadoras de colônias (UFC), divididas em duas etapas por dia. Foram coletadas amostras sangüíneas para análise das concentrações séricas de amilase, lipase, glicose e cálcio, além de contagem leucocitária, imediatamente antes do procedimento, e após 24 e 48 horas. Após a morte dos animais, seguiu-se nova laparotomia para retirada de amostras de líquido peritoneal, linfonodo ileal, pâncreas, baço e fígado, para análise microbiológica e histopatológica. Na comparação simultânea entre os cinco grupos, foram observadas diferenças significativas para contagem leucocitária, na hora 24, e para amilasemia e lipasemia, nas horas 24 e 48 após a indução da PA. Considerando-se somente os grupos de animais em que ocorreu a indução de PA (grupos 2, 3, 4 e 5), não se observou diferença para quaisquer dos parâmetros avaliados. No que concerne à colonização bacteriana dos órgãos, entre estes mesmos quatro grupos, também não se observou diferença. Quando considerados apenas os grupos de animais que receberam BP (grupos 3, 4 e 5), observou-se maior ocorrência de contaminação do linfonodo ileal e de hemorragia pancreática, em comparação aos demais grupos.
