The current master thesis approaches to mental health care and its challenges, at CAPS II and CAPSad II, located in the same territory of the city of Rio de Janeiro, given the relation between severe and persistent mental disorder and use of drugs nowadays. The study focuses on health care in territorially-based services driven to psychosocial care field, its ways of approaching drug use to mental disorders, as well as the political-legal advances and setbacks that impact on it. It is a qualitative, data triangulation research, with a literature review, from a narrative based on theoretical studies, documental analysis related to official documents in the field of mental health, alcohol and other drugs and semi-structured interviews with professionals who work directly with such audience in order to understand the object explained in here. Its conclusion is that the mental health care and drugs addressed to people with severe and persistent mental disorders who make harmful use of drugs has been affected by economic setbacks that bring evident, negative consequences to the service provided to this population. However, part of the difficulty in establishing this care also stems from the hegemonic asylum and prohibitionist culture in the country, which is updated in current government policies, but is also revealed in the barriers imposed by professionals who make up the service network.
