Resumo: Klebsiella pneumoniae has been described as one of the main pathogens related to hospital and community acquired infections due to the increasing mortality. The presence of resistance genes associated with virulence mechanisms, including the ability of biofilm formation, delay the infections treatment. The aim of this present study was to analyze the resistance profile to antimicrobial agents, molecular typing and virulence mechanisms of K. pneumoniae strains isolated from nosocomial infections and river waters. Hospital strains were obtained through LABAC while river isolates strains were collected from four sites of Joana and Maracanã rivers. Multidrug resistance profiles were determined using the disk-diffusion method and polymerase chain reaction (PCR) assays for detection of genes that encode resistance: extended-spectrum beta-lactamase (blaTEM, blaSHV, blaCTX-M e blaTOHO); aminoglycoside-modifying enzymes [aacc2, aacc3 e aacc(6)-Ib-cr]; plasmid mediated quinolones resistance (qnrA, qnrB, qnrD, qnrS e qepA); carbapenemases (blaKpC, blaIMP, blaVIMe blaOXA) and virulence: K1, K2, fimH, mrkD and KfuBc. Additionally, K. pneumoniae MDR strains were subjected to biofilm formation assays on abiotic surfaces, mortality assays using nematode Caenohabiditis elegans as experimental model, investigation of adherence capacity, intracellular viability and persistence using human endothelial CACO-2 cell, in addition to molecular typing by pulsed field gel electrophoresis (PFGE). A total of 45 K. pneumoniae MDR strains were analyzed. Resistance genes were observed in all strains, but three environmental isolates. Hospital strains did not presented resistance gene for carbapenemase while 11 environmental strains harbored at least one resistance genes for different antimicrobial agents classes. Virulence profile demonstrated that all K. pneumoniae MDR strains isolated from nosocomial environment and 21 strains from river environment presented fimH gene. All strains showed biofilm formation capacity at different levels and intensities. PFGE analyzis demonstrated nine different clones, including one pulsotype harbored strains from nosocomial and river environment. Bacterial interaction demonstrated heterogeneity despite none of them ware able to kill Caenorhabditis elegans nematodes after five days infection. All strains were able to adhere on CACO-2 cell at different levels, regardless bacterial concentration and incubation time. The results of the intracellular viability and persistence tests diverge according to execution time and bacterial concentrations. In conclusion, K. pneumoniae strains are heterogeneous and have a high capacity for adaptation. In addition, when associated with different resistance and virulence mechanisms, they are difficulty to treat infections, being necessary to search for new therapeutic options.

A presença de genes de resistência associados a mecanismos de virulência, incluindo a capacidade de formação de biofilme, dificultam o tratamento de infecções. O objetivo do presente estudo foi analisar o perfil de resistência a agentes antimicrobianos, tipificação molecular e os mecanismos de virulência de amostras de K. pneumoniae isoladas de infecções nosocomiais e de águas fluviais. As cepas hospitalares foram obtidas através do LABAC enquanto as cepas de ambientes fluviais foram coletadas em quatro pontos distintos dos rios Joana e Maracanã. Os perfis de multirresistencia foram determinados por técnica de disco-difusão e submetidas à técnica de reação em cadeia da polimerase (PCR) para detecção de genes que codificam resistência: beta-lactamase de espectro estendido (blaTEM, blaSHV, blaCTX-M e blaTOHO); enzimas modificadoras de aminoglicosídeos [aacc2, aacc3 e aacc(6)-Ib-cr]; genes plasmidiais mediando resistência a fluorquinolonas (qnrA, qnrB, qnrD, qnrS e qepA); carbapenemases (blaKpC, blaIMP, blaVIMe blaOXA) e virulência: K1, K2, fimH, mrkD e KfuBc. Adicionalmente, cepas MDR de K. pneumoniae foram submetidas a ensaios de formação de biofilme em superfíceis abióticas diversas, ensaios de mortalidade utilizando o nematódeo Caenohabiditis elegans como modelo expermental, pesquisa da capacidade de aderência, viabilidade intracelular e persistência utilizando célula endotelial humana CACO-2, além da tipificação molecular por técnica de eletroforese em gel de campo pulsado (PFGE). No total, 45 cepas MDR de K. pneumoniae foram analisadas no presente estudo. Foram observadas a presença de genes de resistência em todas as cepas analisadas, exceto três cepas ambientais. Nenhuma das cepas hospitalares apresentou gene de resistência para carbapenemase e 11 cepas ambientais apresentaram genes de resistência de diferentes classes de agentes antimicrobianos concomitantemente. A análise do perfil de virulência demonstrou que todas as cepas MDR de K. pneumoniae isoladas no ambiente hospitalar e 21 cepas do ambiente fluvial apresentaram o gene fimH. Todas as cepas estudadas apresentaram capacidade de formação de biofilme em diferentes níveis e intensidades. Análises realizadas através do PFGE demonstrou a presença de nove perfils clonais distintos, onde, um dos perfis albergou cepas do ambiente hospitalar e fluvial. A interação bacteriana demonstrou uma heterogeneidade, entretanto nenhuma das cepas foi capaz de matar os nematódeos Caenorhabditis elegans após os cinco dias de infecção. Todas as cepas analizadas foram capazes de aderir à superfície da célula CACO-2 em diferentes níveis, independente da concentração bacteriana e do tempo de incubação. Os resultados dos ensaios de viabilidade e persistência intracelular vários de acordo com o tempo de execução e as concentrações bacterianas utilizadas. Foi possível concluir que cepas de K. pneumoniae são heterogêneas e apresentam alta capacidade de adaptação. Multidrug resistance profiles were determined using the disk-diffusion method and polymerase chain reaction (PCR) assays for detection of genes that encode resistance: extended-spectrum beta-lactamase (blaTEM, blaSHV, blaCTX-M e blaTOHO); aminoglycoside-modifying enzymes [aacc2, aacc3 e aacc(6)-Ib-cr]; plasmid mediated quinolones resistance (qnrA, qnrB, qnrD, qnrS e qepA); carbapenemases (blaKpC, blaIMP, blaVIMe blaOXA) and virulence: K1, K2, fimH, mrkD and KfuBc. Additionally, K. pneumoniae MDR strains were subjected to biofilm formation assays on abiotic surfaces, mortality assays using nematode Caenohabiditis elegans as experimental model, investigation of adherence capacity, intracellular viability and persistence using human endothelial CACO-2 cell, in addition to molecular typing by pulsed field gel electrophoresis (PFGE). A total of 45 K. pneumoniae MDR strains were analyzed. Resistance genes were observed in all strains, but three environmental isolates. Hospital strains did not presented resistance gene for carbapenemase while 11 environmental strains harbored at least one resistance genes for different antimicrobial agents classes. Virulence profile demonstrated that all K. pneumoniae MDR strains isolated from nosocomial environment and 21 strains from river environment presented fimH gene. All strains showed biofilm formation capacity at different levels and intensities. PFGE analyzis demonstrated nine different clones, including one pulsotype harbored strains from nosocomial and river environment. Bacterial interaction demonstrated heterogeneity despite none of them ware able to kill Caenorhabditis elegans nematodes after five days infection. All strains were able to adhere on CACO-2 cell at different levels, regardless bacterial concentration and incubation time. The results of the intracellular viability and persistence tests diverge according to execution time and bacterial concentrations. In conclusion, K. pneumoniae strains are heterogeneous and have a high capacity for adaptation. In addition, when associated with different resistance and virulence mechanisms, they are difficulty to treat infections, being necessary to search for new therapeutic options.
