Resumo: The general objective of this thesis was to evaluate the fear of falling in the elderly from a longitudinal perspective and to evaluate the relationship between the fear of falling and the spatio-temporal gait parameters. Its specific objectives were: a) to estimate the incidence and persistence of fear of falling in four years and its risk factors (Article 1); b) to evaluate the association between fear of falling and the spatio temporal gait parameters (Article 2). The research is a subproject of the Frailty Study of the Brazilian Elderly, section Rio de Janeiro (FIBRA-RJ), which began in 2009, the baseline of the study, and reassessed the elderly in 2013 (follow-up phase). The elderly evaluated were clients of a health care provider, residents of the North area of Rio de Janeiro, aged 65 or over. For article 1, 393 elderly people, who participated in both phases, were evaluated. Article 2 used data from 291 elderly people who participated in the follow-up phase and who agreed to participate in gait data collection through GAITRite®. Fear of falling was assessed through the Falls Efficacy Scale (FES-I-BR). In the first article, the association between fear of falling and risk factors were assessed through multivariate analyzes using Poisson regression. The incidence of fear was 33.5% and the risk factors were: use 7 or more medicines and report a worse level of activity. The persistence was 71.3% and the risk factors were: use of 7 or more medicines, history of 1 or 2 falls, diminished walking speed, hearing impairment, cognitive decline, depressive symptoms and poor / very poor self-perception of health. In article 2, fear of falling was associated with the following gait parameters: decrease in speed, cadence, step length and stride length and step time increase. Fear of falling was not associated with variability. It is concluded that the incidence and persistence of fear of falling have different risk factors, and it is a condition that should be evaluated in the elderly, with potential to interfere with gait parameters, increasing the risk of falls, injuries and loss of capacity functional. It is hoped that the results of this thesis will contribute to a greater knowledge of this condition in the elderly, allowing their early detection and appropriate interventions.

Seus objetivos específicos foram: a) estimar a incidência e a persistência do medo de quedas em quatro anos e seus fatores de risco (Artigo 1); b) avaliar a associação entre o medo de quedas e os parâmetros espaço-temporais da marcha (Artigo 2). A pesquisa é um subprojeto do Estudo de Fragilidade em Idosos Brasileiros, seção Rio de Janeiro (FIBRA-RJ), que teve seu início em 2009, a linha de base do estudo, e que reavaliou os idosos em 2013 (fase de seguimento). Os idosos avaliados eram clientes de uma operadora de saúde, moradores da zona Norte do Rio de Janeiro, com 65 anos ou mais. Para o artigo 1, foram avaliados 393 idosos, que participaram das duas fases. O artigo 2 utilizou os dados de 291 idosos, que participaram da fase de seguimento e que aceitaram participar da coleta de dados da marcha, através do GAITRite®. O medo de quedas foi avaliado através da Falls Efficacy Scale (FES-I-BR). No primeiro artigo, a associação entre o medo de quedas e os fatores de risco foi avaliada através de análises multivariadas, através da Regressão de Poisson. A incidência do medo foi de 33,5% e os fatores de risco foram: usar 7 ou mais medicamentos e relatar pior nível de atividade. A persistência do medo de quedas foi de 71,3% e os fatores de risco: usar 7 ou mais medicamentos, história de 1 ou 2 quedas, diminuição da velocidade de marcha, déficit auditivo, declínio cognitivo, sintomas depressivos e autopercepção de saúde ruim/muito ruim. No artigo 2, o medo de quedas foi associado com os seguintes parâmetros da marcha: diminuição da velocidade, da cadência, do comprimento do passo e da passada e aumento do tempo do passo. O medo de quedas não foi associado com a variabilidade. Conclui-se que a incidência e a persistência do medo de quedas possuem fatores de risco distintos, e é uma condição que deve ser avaliada nos idosos, com potencial para interferir nos parâmetros da marcha, aumentando o risco de quedas, lesões e perda de capacidade funcional.
