NEW MANAGEMENT MODELS IN PRIMARY HEALTH CARE AND THE DIFFICULTIES AT WORK

Resumo: The article discusses the reconfiguration of work in Primary Health Care (PHC) based on changes implemented by the Brazilian federal  government since 2017, which have aggravated precarious work conditions. Methodologically it is inspired by Michael Burawoy’s  expanded case ethnography, dialoguing with studies on productive  restructuring and New Public  Management (NGP), linked to the concept of difficulty. Participant  observation and semi-structured  interviews with workers from a health  team from a basic unit of the Unified  Health System (SUS) in Rio de Janeiro  were used. Difficulty has been  characterized as: the constant demand for changes in the organization of daily work and overload due to the  extensive workday, and the demand  for reaching established goals despite  the work conditions. This paper  emphasized the need to continue  analyzing the reconfiguration of health work, considering the impacts of the Constitutional Amendment 95 (2016);  the labor reform (2017); and the  COVID-19 pandemic.

Du point de vue méthodologique, la recherche  s’inspire de l’ethnographie de cas  élargi, selon Michael Burawoy, en  dialogue avec des études sur la restructuration productive et la  Nouvelle Gestion Publique (NGP), liées au concept de la pénibilité au travail.  Nous utilisons l’observation  participante et des entretiens  semistructurées avec des travailleuses  d’un établissement de soins de santé de base du Système de Santé Unifié brésilien (SUS) à Rio de Janeiro. Ont été caractérisées comme types de  pénibilité: l’exigence constante des changements dans l’organisation du  travail quotidien et la surcharge  causée par la longue journée du travail; et la demande pour répondre  des objectifs fixés sans considérer des  particularités du contexte de travail.  L’étude souligne la nécessité de  continuer des analyses de la  reconfiguration du travail en santé en consonance avec des impacts de l’Amendement Constitutionnel 95  (2016) ; de la réforme du travail (2017); et de la pandémie de COVID-19.O artigo trata da reconfiguração do trabalho na Atenção Primária à Saúde (APS) a partir de mudanças implementadas pelo governo federal desde 2017, que agravam as circunstâncias de precarização do  trabalho. Metodologicamente, inspira-se na etnografia de caso ampliado de  Michael Burawoy, dialogando com  estudos sobre a reestruturação produtiva e a Nova Gestão Pública  (NGP), articulados ao conceito de  penosidades. Utilizou-se a observação participante e entrevistas  semiestruturadas com trabalhadoras e uma equipe de saúde de uma unidade  básica do Sistema Único de Saúde  (SUS) do Rio de Janeiro. Caracterizam-se como penosidades: exigência  constante de mudanças na  organização do cotidiano do trabalho e  sobrecarga devido à extensa jornada de trabalho, e cobrança pelo alcance  de metas a despeito das condições de  trabalho.
