Embora o arsenal terapêutico tenha melhorado significativamente nos últimos anos, o desenvolvimento de resistência aos fármacos disponíveis tem sido frequentemente descrito. Estudos recentes relatam crescentes taxas de reincidência, resistência aos quimioterápicos, toxicidade dos fármacos contra tecidos e células normais e efeitos colaterais subjacentes. Portanto, novas abordagens terapêuticas precisam ser exploradas para superar estes obstáculos e o reposicionamento de fármacos tem surgido como uma alternativa promissora. Nesse sentido, muitos estudos têm mostrado que, além da atividade antipsicótica, as fenotiazinas possuem outros efeitos biológicos, incluindo atividade antitumoral in vitro e in vivo contra vários tipos de câncer. Dados recentes do nosso grupo de pesquisa sugerem que um dos mecanismos de indução de morte celular por fenotiazinas em leucemia envolve a modulação da autofagia por esses fármacos. Entretanto, foi mostrado que o derivado fenotiazínico prometazina induziu autofagia associada à apotose em modelo de leucemia mieloide crônica (LMC), mas a indução de autofagia pelo derivado tioridazina em modelo de leucemia linfoblástica aguda (LLA) protegeu as células da morte celular, e os mecanismos responsáveis por tais efeitos não estão esclarecidos. Assim, o objetivo deste trabalho foi investigar o papel da autofagia induzida pelo derivado fenotiazínico de uso clínico, em diferentes tipos de leucemia, a fim de compreender os mecanismos moleculares que promovem morte ou a sobrevivência celular. Além disso, pretende-se avaliar a modulação da autofagia como possível alvo terapêutico para a quimioterapia em leucemia. Nos ensaios de viabilidade celular, os quatro derivados fenotiazínicos de uso clínico tioridazina (TR), prometazina (PMTZ), trifluoperazina (TFP) e clorpromazina (CPZ) exibiram citotoxicidade de forma concentração dependente. Dentre eles, TR foi o derivado mais citotóxico nos diversos modelos de leucemias estudados, sendo então selecionado para as investigações subsequentes. Em células K562 (LMC), TR induziu o aumento do fluxo autofágico, determinado pelo aumento de LC3-II e SQSTM1 (p62). Além disso, a inibição da autofagia por bafilomicina A1 diminuiu a citotoxicidade de TR, ao mesmo tempo que o indutor de autofagia rapamicina potencializou seu efeito citotóxico. Dessa forma, diferentemente do que foi observado em modelo de LLA, TR induziu autofagia associada à morte celular em modelo de LMC, ressaltando o duplo papel da autofagia já descrito na literatura, ora atuando pró-morte, ora pró-sobrevivência. Este resultado é uma indicação de que o papel da autofagia pode ser devido ao subtipo e característica específica da leucemia sobre o estímulo das fenotiazinas, independentemente de ser TR ou PMTZ. Embora mais estudos sejam necessários para esclarecer tais diferenças de resposta à TR na LLA e LMC. A investigação dos mecanismos moleculares revelou que a ação já bem documentada de TR como antagonista em dopaminergic receptors (DR), um tipo de receptor acoplado à proteína (GPCR), está relacionada à indução da autofagia associada à morte celular em LMC. Foi observado que o aumento LC3-II e a citotoxicidade de TR foram potencializados pelo antagonista de DR haloperidol e reduzidos pelo agonista de DR dopamina.TR provavelmente inibe D2R (GPCR ?i/o), uma vez que o inibidor da adenilil ciclase (SQ 25.536) protegeu do seu efeito citotóxico, mostrando que vias de sinalização dowstream à produção de AMPc podem estar associadas à morte celular. Adicionalmente mostramos que apesar da diminuição da via PI3K/AKT/mTOR ser importante para a citotoxicidade de TR, não conseguimos correlacionar o seu efeito com a modulação do receptor dopaminérgico, pois experimentos adicionais são necessários para esclarecer os efeitos observados até o momento. Em conjunto, esses resultados mostraram que o efeito citotóxico de TR em LMC está relacionado à indução de autofagia associada à apoptose e que sua ação farmacológica sobre DR participa deste mecanismo de morte celular.Leukemias comprises a set of heterogeneous hematological neoplastic diseases that occupy the 10th position among the cancer types with the highest mortality rates in the world. Although the therapeutic arsenal has improved significantly in recent years, the development of resistance to available drugs has been often described. Recent studies have reported increasing rates of relapse, drug resistance, drug toxicity against normal tissues and cells, and other side effects. Therefore, novel therapeutic approaches need to be explored to overcome these challenges and the drug repositioning has emerged as a promising alternative. In this regard, several studies have shown that, in addition to antipsychotic and neuroleptic activity, phenothiazines have other biological effects, including antitumor activity in vitro and in vivo against various types of cancer. Recent data from our research group suggest that one of the mechanisms of phenothiazine-induced cell death in leukemia cells involves the modulation of autophagy. It was shown that the phenothiazine derivative promethazine induced apotosis-associated autophagy in a chronic myeloid leukemia (CML) model, but on the contrary thioridazine-induced autophagy protected acute lymphoblastic leukemia (ALL) cells from death. The mechanisms responsible for these effects remain unclear. Thus, the aim of this work was to investigate the role of autophagy induced by the phenothiazine derivative used in Medicine on the viability of different types of leukemia cells, in order to understand the alterations leading to cell death or survival. Furthermore, we intend to evaluate the modulation of autophagy as a potential therapeutic target for chemotherapy in leukemia. In cell viability assays, the four clinically used phenothiazine derivatives thioridazine (TR), promethazine (PMTZ), trifluoperazine (TFP) and chlorpromazine (CPZ) exhibited concentration-dependent cytotoxicity. Among them, TR was the most cytotoxic derivative, being selected for subsequent studies. In K562 cells (LMC), TR increased the autophagic flux, assessed by an increase in LC3-II and SQSTM1 (p62) levels. Furthermore, the inhibition of autophagy by bafilomycin A1 decreased the cytotoxicity of TR, whereas the autophagy inducer rapamycin potentiated its cytotoxic effect. Thus, differently from what was observed in the ALL model, TR induced autophagy associated with cell death in the CML model, highlighting the double role of autophagy already described, either pro-death or pro-survival. This result is na indication that the role of autophagy may be due to the subtype and specific characteristic of leukemia on the stimulus of phenothiazines, regardless of whether it is TR or PMTZ. Although more studies are needed to clarify such differences in response to TR in ALL and CML. Investigation of molecular mechanisms revealed that the well-documented action of TR as an antagonist at dopaminergic receptors (DR), a type of protein-coupled receptor (GPCR), is related to the induction of autophagy associated with cell death in CML. It was observed that the LC3-II increase and TR cytotoxicity were potentiated by the DR antagonist haloperidol and reduced by the DR agonist dopamine.TR probably inhibits D2R (GPCR ?i/o), since the adenylyl cyclase inhibitor (SQ 25.536 ) protected from its cytotoxic effect, showing that downstream signaling pathways to cAMP production may be associated with cell death. Additionally, we showed that although the decrease in the PI3K/AKT/mTOR pathway is important for TR cytotoxicity, we were unable to correlate its effect with dopaminergic receptor modulation, as additional experiments are needed to clarify the effects observed so far.
