Mental health care - the view of family members

Resumo: The objective of this study is to understand the process of restructuring psychiatric care in Brazil from the point of view of family members. The settings used in this study were the Associação Franco Basaglia (in São Paulo) and the Associação de Amigos, Familiares e Doentes Mentais do Brasil (AFDM - in Rio de Janeiro). Data collection included the analysis of documents about the creation and organization of both associations and semi-structured interviews. There were ten interviews with relatives who were also members of the board of the Associations. The investigation showed that, for the family members of AFDM, besides the difficulty in living near the mental patients, the lack of trust in the assistance provided by our services, and the fact that the State does not do its role in providing health care to the population, there is also the possibility of closure of the local psychiatric hospitals. For the families of AFB, the burden of caring for their mental patients is mitigated by the partnership with the services and professionals who give assistance to the mental patients. The problem may be solved by the construction and development of State services and institutions, training centers and health professionals, focusing on the social and historical context faced by the families of mental patients.

Los escenarios utilizados fueron la Asociación Franco Basaglia (SP) y la Asociación de Amigos, Familiares y Enfermos Mentales de Brasil (RJ). La colecta de datos incluyó búsqueda en documentos de la fundación y organización de ambas asociaciones y entrevistas semiabiertas. Se realizaron diez entrevistas con familiares a cargo de la dirección de las Asociaciones. La investigación ha revelado que, para los familiares de la AFDM, la dificultad en convivir con la enfermedad yace en la falta de confianza en la atención que ofrecen los servicios y que la convicción de que el Estado no ejerce bien su papel en cuanto a atención a la salud de la población aumenta con la posibilidad de que cierren los hospitales psiquiátricos. Para los familiares de la AFB la sobrecarga de cuidado se atenúa con la seguridad de una alianza entre los servicios y profesionales en la asistencia brindada al enfermo mental. La superación tendrá lugar con la construcción y desarrollo de un trabajo entre el Estado, los servicios, las instituciones de formación y los profesionales de salud, orientado a la realidad social e histórica vivida por las familias en la asistencia al enfermo mental.Este estudo tem como objetivo compreender, na visão dos familiares, o processo de reestruturação da assistência psiquiátrica no Brasil. Os cenários utilizados foram a Associação Franco Basaglia (AFB) de São Paulo e a Associação de Amigos, Familiares e Doentes Mentais do Brasil (AFDM) do Rio de Janeiro. A coleta de dados constou de consulta aos documentos relativos à criação e organização das duas associações e entrevistas semi-estruturadas. Foram realizadas dez entrevistas com familiares que ocupam cargos de direção nas Associações. A investigação revelou que, para os familiares da AFDM, a dificuldade na convivência com a doença, a descrença no atendimento prestado pelos novos serviços, a certeza de que o Estado não vem desempenhando o seu papel na assistência à saúde da população são exacerbadas pela possibilidade de fechamento dos hospitais psiquiátricos. Para os familiares da AFB, a sobrecarga do cuidado é amainada pela certeza da parceria com os serviços e profissionais na assistência prestada ao doente mental.
