Alterações da mobilidade do tronco em idosos com dor lombar aguda

Resumo: Background: The aging process is comprised of a series of changes, which may be accompanied by increased musculoskeletal complaints and symptoms. With increasing life expectancy, degenerative chronic diseases and their complications become more frequent, often accompanied by pain and limitations. Among the elderly there is the back pain, relevant complaint and with great impact on functional performance. However studies on low back pain and aging are still limited. Aim: The objective of this study was to analyze the trunk mobility disorders in elderly patients with acute low back pain, comparing clinical and functional outcomes in elderly patients with and without a change in mobility of the trunk. Methods: This study is derived from the study Back Complaints in the Elders in Brazil (BACE-B), whose main objectives and details of the protocol have been published previously. The mobility of the trunk was evaluated in two ways: the trunk flexion by Fingertip-to-Floor Test and the lateral flexion by Lateral flexion finger-floor-distance adapted categories. Pain intensity was evaluated by the numerical pain scale; functionality by Time Up and Go (TUG); and the inability by Rolland Morris questionnaire. The statistics were performed by descriptive analysis of the data and to compare the differences between groups we used the Mann Whitney U-test, with significance level of 5%. Results: The study included 532 elderly people, average age 69.04 years, most being women (85.7%) and low level of education (64.1%). Of the elderly who participated in the study, 53% had anterior flexion mobility alteration and 53.1% had lateral flexion mobility change. Elderly with trunk mobility changes, both in flexion and trunk side, had worse levels of pain, disability and functionality, p <0.001. Conclusion: The results demonstrated the association of trunk mobility changes with important clinical outcomes in the evaluation of elderly patients with low back pain, such as pain intensity, disability and functionality. However, these are initial results and further studies are needed to investigate the changes of mobility in elderly patients with acute low back pain.

Com o aumento da expectativa de vida, as doenças crônicas degenerativas e suas complicações tornam-se mais frequentes, muitas vezes acompanhadas pela dor e suas limitações. Entre os idosos destaca-se a dor lombar, queixa relevante e com grande impacto no desempenho funcional. No entanto os estudos sobre dor lombar e envelhecimento ainda são limitados. Objetivo: O objetivo desse estudo foi analisar as alterações de mobilidade do tronco em idosos com dor lombar aguda, comparando desfechos clínicos e funcionais em idosos com e sem alteração de mobilidade do tronco. Materiais e métodos: O presente estudo é derivado do estudo Back Complaints in the Elders no Brasil (BACE-B), cujos principais objetivos e detalhes do protocolo já foram publicados previamente. A mobilidade do tronco foi avaliada de duas maneiras: flexão anterior de tronco pelo teste distância dedo-chão e flexão lateral de tronco pelo teste distância dedo chão adaptado em categorias. A intensidade da dor foi avaliada pela escala numérica da dor; a funcionalidade pelo Time Up and Go (TUG); e a incapacidade pelo questionário Rolland Morris. A estatística foi realizada pela análise descritiva dos dados e para comparação das diferenças entre grupos foi utilizado o teste Mann Whitney-U, com nível de significância de 5%. Resultados: Participaram do presente estudo 532 idosos, média de idade 69,04 anos, sendo a maioria de mulheres (85,7%) e baixo nível de escolaridade (64,1%). Dos idosos que participaram do estudo, 53% apresentaram alteração de mobilidade de flexão anterior e 53,1% apresentaram alteração de mobilidade de flexão lateral. Idosos com alterações da mobilidade do tronco, tanto na flexão anterior e lateral de tronco, apresentaram piores níveis de dor, incapacidade e funcionalidade, p<0,001. Conclusão: Os resultados demonstraram a associação das alterações de mobilidade do tronco com desfechos clínicos importantes na avaliação de idosos com dor lombar, como intensidade da dor, incapacidade e funcionalidade.
