OBJETIVO: Comparar a mobilidade diafragmática, a função pulmonar e o desempenho cardiorrespiratório entre indivíduos com diferentes graus de incapacidade associados à lombalgia crônica e controles sem histórico de lombalgia. MÉTODOS: Trata-se de um estudo observacional de caráter transversal, com amostra composta por 33 indivíduos de ambos os sexos, entre 20 e 59 anos, divididos no grupo lombalgia leve/moderada e grave de acordo com o índice de deficiência de Oswestry (IDO), e um grupo de indivíduos sem lombalgia. Em tais indivíduos foram avaliados a idade, altura, peso e circunferência abdominal através da ficha de avaliação e o nível de atividade física pelo questionário internacional de atividade física. Também foram avaliadas à a mobilidade diafragmática através da ultrassonografia, espirometria e o teste de caminhada de seis minutos para aferir o desempenho cardiorrespiratório. RESULTADOS: Os grupos foram homogêneos em relação às características de sexo, idade, peso, altura e grau de atividade física. No entanto, o grupo lombalgia grave apresentou menor volume expiratório forçado no primeiro segundo (VEF1) (1,98±0,7l/s), capacidade vital forçada (CVF) (2,66±0,63l) e desempenho cardiorrespiratório (270±119m) quando comparado ao grupo controle, 3,24±0,66l/s; 3,91±0,7l e 635±341,6m respectivamente. Em relação à mobilidade diafragmática, ao pico de fluxo expiratório (PFE) e à relação VEF1/CVF não houve diferença entre os grupos. CONCLUSÃO: Indivíduos com maior incapacidade associada a lombalgia crônica apresentaram menor desempenho espirométrico, com menores valores de VEF1 e CVF, e menor desempenho cardiorrespiratório. No entanto, a mobilidade diafragmática não se alterou conforme a incapacidade associada a lombalgia.INTRODUCTION: Biomechanical alterations associated with chronic low back pain may lead to impairment in respiratory mechanics and cardiopulmonary performance; however, this association still needs to be elucidated. OBJECTIVE: It was to compare diaphragmatic mobility, pulmonary function and cardiorespiratory performance among individuals with different degrees of disability associated with chronic low back pain and controls with no history of low back pain. METHODS: This was a cross-sectional observational study with a sample composed of 33 individuals of both genders, aged between 20 and 59 years, divided into the group of light/moderate and severe low back pain according to the Oswestry disability index (ODI), and a group of individuals without low back pain. In such individuals, the age, height, weight and waist circumference were assessed through the evaluation form, and the level of physical activity by the international physical activity questionnaire (IPAq). The diaphragmatic mobility was also assessed by ultrasonography, as well as spirometry and the six-minute walk test to assess cardiorespiratory performance. RESULTS: The groups were homogeneous in relation to the characteristics of gender, age, weight, height and degree of physical activity. However, the low back pain group had lower forced expiratory volume in the first second (FEV1) (1.98±0.7l/s), forced vital capacity (FVC) (2.66±0.63l) and cardiorespiratory performance (270±119m) when compared to the control group, 3.24±0.66 l/s; 3.91±0.7l and 635±341.6 m respectively. However, the diaphragmatic mobility, the peak expiratory flow (PEF) and the FEV1/FVC ratio, there was no difference between the groups. CONCLUSION: Individuals with greater disability associated with chronic low back pain had lower spirometric performance, with lower values of FEV1 and FVC, and lower cardiorespiratory performance.
