Defesa : Curitiba, 30/09/2019Inclui referências: p. 91-96Resumo: A partir da orientação psicanalítica lacaniana e diante de impasses na clínica, nos perguntamos, primeiramente, como identificar a psicose na criança, considerando a hipótese de que é importante estabelecer diagnóstico estrutural para pensar a direção do tratamento. Deparamo-nos com atual divergência entre psicanalistas que atendem crianças, quanto ao estatuto do diagnóstico de psicose. De um lado, considera-se que a psicose pode ser curada nessa fase do desenvolvimento denominada infância, o que, em outras palavras, quer dizer encaminhar a estruturação da criança para uma neurose. De outro, critica-se essa posição, defendendo que não há possibilidade de mudança estrutural e que a direção de cura se dá dentro das possibilidades e limites de cada estrutura. Esse impasse teórico não é solucionável, pois ambas as posições podem ser "demonstradas" pela via da exposição do caso clínico. Assim, para avançar na questão acerca do estatuto do diagnóstico estrutural de psicose na criança, foi realizado um retorno a Lacan, situando seu percurso em direção à noção de estrutura psicótica. Questões de ordem epistemológica então se destacaram, tais como a noção de verdade da loucura humana, em sua indissociabilidade a uma posição ética, assim como a noção de causalidade da psicose. Para Lacan, não se trata de uma verdade a ser demonstrada pela via da exposição do caso clínico, e tampouco de uma hipótese a ser confirmada, porque não se trata de um conceito materialista, nem idealista. Trata-se de um conceito abstrato, ficcional, que visa orientar uma práxis. Diante disso, não se pode falar em causa da psicose; a foraclusão do Nomedo- pai não se refere à sua gênese. Notamos que ao mesmo tempo em que Lacan desenvolve uma teoria das psicoses, ele delimita um campo de saber, com epistemologia específica, no intuito de possibilitar um tratamento psicanalítico possível para as psicoses. Podemos concluir que a divergência acerca da psicose na criança é de ordem epistemológica, de forma que uma não pode refutar a outra, a não ser pela via de argumentos acerca dos fundamentos do saber psicanalítico. Assim, esse retorno a Lacan promoveu um deslocamento da questão inicial para a implicação da concepção da psicose enquanto estrutura no processo diagnóstico, ou seja, como o analista faz diagnóstico e com que finalidade? O que foi possível responder neste momento é que não se trata de identificar o diagnóstico de psicose, como se ele fosse localizável, mas sim de levantar hipóteses diagnósticas visando à construção de coordenadas para a condução do tratamento; nunca no sentido de uma profilaxia, seja com crianças ou adultos. Trata-se de, na relação transferencial com o analisante, localizar um princípio estruturante da forma com que ele se coloca na linguagem. Se a hipótese do analista for de que a relação do sujeito com a linguagem não está alinhada à norma fálica, ele tomará o cuidado ainda maior de não intervir desde uma posição encarnada de saber. Trata-se, então, de acompanhar a criança, estando atento ao que ela pode inventar de maneira singular no percurso de seu tratamento. Palavras-chave: Psicose infantil. Diagnóstico psicanalítico. Epistemologia psicanalítica.Abstract: From impasses in the psychoanalytic clinic with children, we wondered how we could identify the diagnosis of psychosis. That question was based on the hypothesis that it is important to establish structural diagnosis in order to think of a direction for the treatment. From that on, we found out about a divergence among psychoanalysts regarding the status of the psychotic structure in children. Some of them defend the idea that psychosis can be cured during this time we call childhood, which means that it could be directed to a neurotic structure. Others, on the other hand, criticize that idea, arguing that not only changing the structure is not possible, but also it is not the analytic treatment's goal. For them, the cure's direction can only be thought within the possibilities and limits of each structure. This theoretical impasse about psychosis in children could not be solved, since both positions can be "demonstrated" trough case reports. That way we were led to Lacan's work from his theses to his third seminar, to draw his way towards the notion of psychotic structure. We realized that Lacan discussed all the time about epistemological issues, such as the notion of truth about madness and its close relation to an ethical position, as well as the notion of psychosis' causality. It turns out that his psychoanalyst concepts' notion of truth is not to be demonstrated, nor it is a hypothesis to be confirmed, since it is not a materialistic, nor an idealistic concept. It is an abstract, fictional, concept that aims to orient the analyst. Knowing that, we can affirm that from this theory we cannot talk about psychosis causality; the foreclosure of the father is not meant to tell us where the psychotic structure begins. We came to understand that while Lacan develops a theory of psychosis, he also sets its limits, with the intent to create a possible treatment for psychosis. Our question then moves to others that are related to psychoanalytic epistemology, since it turns evident that the theoretical impasse we first faced is all about that. That being said, we also had the will to get to the question about what are the implications of this theory, with its limits, on the diagnosis' process, and why is it any useful for the analyst? What we could answer is that diagnosis will always have the status of a hypothesis, and its proposal is to give the analyst some coordinates for the treatment, but never as a prophylaxis. In transference with the pacient, the analyst should locate how to interfere, noticing how that patient fits into language and relates to it. If the subject's relation to language is not aligned to a phallic norm, the analyst will take double care not to intervene from a position full of whole knowledge. The treatment will then be about going with the child, paying attention to what he/she can invent in her/his unique way. Keywords: Child psychosis. Psychoanalytic diagnosis.
