Resumo: Background: To evaluate the association between haematological and biochemical components in patients with severe visceral leishmaniasis (VL). To identify specific factors of the immunopathogenesis of severe VL. Methods: Cross-sectional data collection from medical records between 2008 – 2013 in the center of reference of VL in the State of Sergipe – University Hospital. We studied 34 individuals, including 11 asymptomatic individuals with positive serology for VL, 10 patients with classic VL and 13 patients with severe VL. Patients with severe disease were classified according with risk score of death. Results: the male was more frequent in groups classic VL (60%, n = 6) and severe VL (77%, n = 10). Individuals with severe disease had high frequency of dyspnea, spontaneous bleeding and bacterial infections, unlike patients with asymptomatic leishmaniasis and classic VL. Hematologic levels of asymptomatic individuals were significantly different from those patients with severe VL and classic form, with the exception of the number of monocytes, which was not significant among the 3 groups. Aminotransferase and bilirubin, although they increase, were not statistically different. Conclusion: Decrease in haematological parameters, mainly leucopenia and neutropenia are associated with disease severity

Para identificar fatores específicos da imunopatogênese da LV grave. Método: Estudo transversal. Revisão de prontuários entre 2008 – 2013, no centro de referência de LV no Estado de Sergipe – Hospital Universitário. Foram estudados 34 indivíduos, incluindo 11 indivíduos assintomáticos com sorologia positiva para LV, 10 pacientes com calazar clássico e 13 pacientes com LV grave. Os pacientes graves foram classificados de acordo com o escore de risco de morte. Resultados: O sexo masculino foi mais freqüente nos grupos calazar clássico (60%, n=6) e calazar grave (77%, n=10). Indivíduos com calazar grave tiveram alta frequência de dispnéia, sangramentos espontâneos e infecção bacteriana, diferente dos pacientes com calazar clássico e indivíduos assintomáticos. Os valores hematológicos dos indivíduos assintomáticos foram significantemente diferentes daqueles pacientes com LV clássico e grave, com exceção do número de monócitos, que não foi significante entre os 3 grupos. Aminotransferases e bilirrubinas, apesar de estarem aumentadas, não foram estatisticamente diferentes. Conclusões: Diminuição dos parâmetros hematológicos, principalmente leucopenia e neutropenia estão associados à gravidade da doença.
