Fatores associados à sifilis congênita em Sergipe

Resumo: | Objectives: to identify associated factors with congenital syphilis in Sergipe, to analyze the antenatal assistance for the mothers of children with congenital syphilis and to identify the follow up of these children. Methods: transversal study accomplished in the follow up outpatient in motherhoods of Sergipe between November 2017 and April 2018 with one hundred twelve mothers with their children. After the interview with the mothers, we verify the children’s medical records, the abstracts of maternity hospital discharge and the historic of the mother’s antenatal care. The analyzed data were inserted in the software Microsoft Excel and the bivariate analyze was performed in the program Epi Info version 7.2.1.0. Results: Most women is between 20 and 24 years old (30/27.52%), 58(52.79%) live in countryside, 62(56.36%) were married. 71(63.39%) of them had studied more than eight years. The sexual activity with more than one partners in the 12 months before pregnancy was reported by 14(13.46%). The inconstant use of condom was reported by 59(53.15%); 18(16.21%) reported diagnosis of sexually transmitted infection before pregnancy. 28(25.22%) reported not knowing the severity of the disease during the pregnancy e 27(24.32%) not knowing the importance of the partner being treated. Almost the total did perform treatment and only one third had diagnosis of syphilis in the first trimester. Among who did perform treatment, 88(78.57%) were appropriate. Among the 10 cases who did not perform treatment, the main reason were service flow failure (6/60%). Most children (77/74.04%) were born normal, 106 (97.25%) did perform one complementary exam at least, 25 (22.32%) did not perform the non-treponemal testing in the spinal tap and 72 (67.92%) were asymptomatic. Among the 99 cases registered of the children’s treatment, 88(88.89%) neonates received appropriate treatment. The bivariate analyze showed significance between some study variables. Conclusion: it is very important to understand the syphilis dynamics in the state, because knowing the real magnitude of this infection and the reality of the assistance, arise the opportunity to start strategies for the improvement in the pregnancy assistance and in the prevention of the vertical transmission. |

Método: estudo transversal realizado nos ambulatórios de seguimento em maternidades de Sergipe entre novembro de 2017 e abril de 2018 com 112 mães com suas crianças. Após a entrevista com as mães, verificou-se os prontuários das crianças, resumo de alta da maternidade e o histórico do pré-natal da mãe. As variáveis foram divididas nas categorias: Aspectos sócio-demográficos-econômicos; exposição de risco materno; antecedentes obstétricos da mãe e exposição da criança. Os dados foram armazenados no software Microsoft Excel e a análise bivariada foi realizada no software Epi Info versão 7.2.1.0. Resultados: A maioria das mães possuíam idade entre 20 e 24 anos (30/27,52%), moravam no interior (58/52,79%), eram casadas (62/56,36%) e possuíam mais de 8 anos de estudo (71/63,39%). O contato sexual com mais de um parceiro nos 12 meses antes da gestação foi referido por 14(13,46%), o uso inconstante do preservativo foi observado em 59 casos (53,15%); 18(16,21%) relataram diagnóstico de IST anterior a gestação. 28(25,22%) afirmaram não ter ciência da gravidade da doença durante a gestação e 27(24,32%) não sabiam da importância do parceiro ser tratado. Quase a totalidade realizou pré-natal e apenas um terço teve diagnóstico da sífilis no primeiro trimestre. Das que realizaram tratamento, 88 (78,57%) tiveram esquema considerado adequado. Dos 10 casos que não realizaram tratamento, o principal motivo observado foi falha no fluxo do serviço (6/60%). A maioria (77 /74,04%) das crianças nasceram a termo, 106 (97,25%) fizeram pelo menos um exame complementar, 25 (22,32%) não realizaram o teste não treponêmico no líquor e 72 (67,92%) foram assintomáticas. Dos 99 casos com registro de tratamento da criança, 88(88,89%) recém nascidos receberam tratamento adequado. A análise bivariada mostrou relação significativa entre algumas variáveis do estudo.
