Today, the improvement of technology in the medical field has allowed diseases to be treated at home, giving patients the benefit of not being constantly in a hospital environment, where are high the health risks of patients, caregivers and family members. The concept of Home Care has been contributing to the patients stay length reduction at the hospital and, consequently, an available beds increase number. This implies reducing costs for hospitals without raising it to patients. For each patient who leaves the hospital, about 60 empty beds are freed at the end of the month. In Brazil, it is increasingly usual to use this type of service, which grows around 5% a year. In this scenario, concepts such as Smart Cities and Internet of Things (IoT) have stood out as a trend of ICT that allowed the creation of new, more accurate and more information rich treatments. The CuidSE is a smartphone application that aims to help elder patients and Home Health Care professionals to comply with their commitments more rigorously by placing this information at the "palm of the hand." Through systematic and market reviews it was possible to explore the subject and explore features present in home health care applications. This is a subject still few explored by computing area, so this research is exploratory. Its need to contribute to practical purposes classifies it as an applied research nature. To manage the project activities, the Scrum framework was aided by the Trello tool
