Pacientes em hemodi?lise que apresentam fun??o renal residual, apresentam menores n?veis de citocinas inflamat?rias circulantes e melhores avalia??es cardiovasculares. Postula-se tamb?m que este subgrupo de pacientes apresentem uma progress?o menos acentuada da disfun??o osteomuscular. Entretanto, a influ?ncia da diurese residual sobre a fun??o ?ssea e muscular de pacientes em hemodi?lise ainda precisa ser estudada. Objetivo: Avaliar a rela??o da diurese residual com a fun??o ?ssea e muscular de pacientes com DRC em hemodi?lise. M?todos: Por meio de um estudo transversal, pacientes em hemodi?lise foram submetidos ao exame de Absormetria Radiol?gica de Dupla Energia (DEXA), para registro da densidade mineral ?ssea e do ?ndice relativo de massa muscular esquel?tica). Com base no volume de diurese coletado em 24h, os pacientes foram estratificados em an?ricos (diurese ? 100 mL/dia) ou n?o an?ricos (diurese > 100 mL/dia). Os valores de densidade mineral ?ssea foram correlacionados ao volume de diurese em 24 h pelo teste de Spearman. Os dados do DEXA e a propor??o de indiv?duos osteop?nicos e sarcop?nicos entre os grupos An?rico e N?o An?rico foram comparados pelos testes Mann Whitney e qui-quadrado, respectivamente. A influ?ncia da aus?ncia de diurese residual sobre presen?a de sarcopenia e osteopenia foram analisadas por modelos de regress?o log?stica bin?ria simples e ajustados por idade, sexo e tempo de tratamento em di?lise. Signific?ncia estat?stica foi considerada quando p < 0,05. Resultados: Noventa e dois pacientes, com m?dia de idade de 54,4 anos (IC95% 51,3 ? 57,4), foram avaliados (47 an?ricos e 45 n?o an?ricos). O volume de diurese di?rio apresentou correla??o significativa com a densidade mineral ?ssea (r=0,24; p=0,02). O grupo an?rico apresentou menor densidade mineral ?ssea (p=0,007) e maior propor??o de indiv?duos com osteopenia (p=0,025), em compara??o ao n?o an?rico. N?o houve diferen?as entre os grupos na propor??o de indiv?duos com sarcopenia (p=0,129). Pacientes an?ricos apresentaram um risco 2,77 vezes maior de apresentar sarcopenia e 2,60 vezes maior de apresentar osteopenia, independente do sexo, idade e tempo de di?lise. O sexo foi a outra vari?vel preditora para presen?a de sarcopenia, tendo o sexo masculino um risco de 3,30 vezes maior. Hemodialysis patients who have residual renal function, have lower levels of circulating inflammatory cytokines and better cardiovascular assessments. Also, it has been postuling that this subgroup of patients has a less marked progression of musculoskeletal dysfunction. However, the influence of residual diuresis on bone and muscle function in hemodialysis patients is unknow. Objective: To evaluate the relationship between residual diuresis and bone and muscle function in hemodialysis patients. Methods: in this cross-sectional study, hemodialysis patients were submitted to Dual Energy Radiological Absormetry (DEXA), to record bone mineral density and the relative skeletal muscle mass index. Based on the volume of diuresis collected in 24 hours, patients were stratified into anuric (diuresis ? 100 mL / day) or non-anuric (diuresis > 100 mL / day). The values of bone mineral density were correlated with the volume of diuresis in 24 h using the Spearman test. DEXA data and the proportion of osteopenic and sarcopenic individuals between the Anuric and Non-Anuric groups were compared using the Mann Whitney and chi-square tests, respectively. The influence anuria on the presence of sarcopenia and osteopenia were analyzed using simple binary logistic regression models and adjusted for age, sex and length of hemodialysis treatment. Statistical significance was set in p <0.05. Results: Ninety-two patients, with a mean age of 54.4 years (95% CI 51.3 - 57.4), were evaluated (47 anuric and 45 non-anuric). The volume of diuresis showed a significant correlation with bone mineral density (r = 0.24; p = 0.02). The anuric group had lower bone mineral density (p = 0.007) and a higher proportion of individuals with osteopenia (p = 0.025), compared to non-anuric. There were no differences between groups in the proportion of individuals with sarcopenia (p = 0.129). Anuric patients were 2.77 times more likely to have sarcopenia and 2.60 times more likely to have osteopenia, regardless of gender, age and time on dialysis. Gender was the other predictive variable for the presence of sarcopenia, with males having a risk of 3.30 times greater.
