Resumo: Researchs on surgical adhesives for the treatment of corneal perforations are many and show its effectiveness on maintaining ocular integrity and restraining inflammatory reaction. They show the adhesive`s properties favoring epithelization and its anti-inflammatory action. Thus, we compared the surgical techniques and the effects of N-butyl 2-cyanoacrylate and the bovine amniotic membrane (previously preserved in 99% glycerin under room temperature), aiming to offer technique for the repair of corneal perforations. For such, perforating keratectomies were made on the corneas of 60 rabbits equally divided in 4 groups. Group 1 (G1) was treated using N-butyl 2- cyanoacrylate applied on the lesions; group 2 (G2) received the amniotic membrane over the lesions and then the N-butyl 2-cyanoacrylate; in group 3 (G3), the same procedure as in group 2 was made, with additional coating with a amniotic membrane as a patch sutured to the corneal limbus; while group 4 (G4) was treated with keratoplasty with amniotic membrane covered by the same membrane patch, sutured to the corneal limbus. Daily ophthalmic examinations were made to verify parameters related to corneal, anterior chamber and lens condition, as well as corneal epithelization and the time during which the adhesive remained in place. Histopathology and histomorphometry were made at 2, 7, 15, 21 and 30 days post-operation (PO) for: the degree of corneal epithelization, leukocytes and fibroblasts presence, vessels presence, edema, and thickness of the epithelium and the stroma. Photophobia, blefarospasm, ocular discharge, conjunctival hyperemia and chemosis were observed, decreasing at 7 days PO. Anterior synechia occured in 2 animals in G1. Corneal edema occured in the perilesional area in all groups. Lens opacity was observed in G1, G2, and G3. In G2 and G3, the membrane acted as a barrier to the aqueous humour, keeping a dry surface to the application of the adhesive as well as preventing its contact with the intraocular structures and, when associated to the adhesive, it favored the repair process. The surgical technique associating the amniotic membrane to the N-butyl 2-cyanoacrylate was the easiest procedure to be implemented and more effective on the repair of perforated corneas in comparison to both the ones using only the adhesive or the membrane. Therefore, the association of the membrane to the adhesive can be indicated in the treatment of 2mm corneal perforations.

Therefore, the association of the membrane to the adhesive can be indicated in the treatment of 2mm corneal perforations.As pesquisas envolvendo o uso de adesivos cirúrgicos na terapia de lesões penetrantes na córnea são amplas e demonstram sua eficiência na manutenção da integridade ocular e a reação inflamatória causada pelo seu uso. Os vários estudos envolvendo a membrana amniótica denotam suas propriedades favoráveis à epitelização e à sua ação anti-inflamatória. Diante das observações, compararam-se técnicas cirúrgicas e os efeitos do n-butil 2-cianoacrilato e da membrana amniótica bovina, conservada em glicerina 99% em temperatura ambiente, visando propor técnica para reparação de lesões de córneas perfuradas. Para tanto, foram feitas ceratectomias penetrantes nas córneas de 60 coelhos distribuídos em quatro grupos de número igual. O grupo 1 foi tratado com aplicação de n-butil 2-cianoacrilato sobre a lesão; o grupo 2 com aplicação da membrana amniótica sob a córnea lesada e de n-butil 2-cianoacrilato sobre a lesão; o grupo 3 recebeu o mesmo tratamento instituído ao grupo 2, adicionando-se recobrimento com bandagem de membrana amniótica suturada ao limbo e, o grupo 4 foi tratado por ceratoplastia com membrana amniótica e recobrimento com bandagem constituída de membrana idêntica, suturada ao limbo. Foi feita avaliação oftálmica diariamente para verificar os parâmetros relacionados à condição corneal, câmara anterior e lente, além da epitelização da córnea e do tempo de permanência do adesivo. Histopatologia e histomorfometria foram feitas aos 2, 7, 15, 21 e 30 dias após a realização da cirurgia avaliando-se o grau de epitelização da córnea, a presença de leucócitos, edema, vasos, fibroblastos, além das espessuras do epitélio e do estroma. Foram observados fotofobia e blefarospasmo, secreção ocular, hiperemia conjuntival e quemose com atenuamento aos sete dias da cirurgia. Houve a formação de sinéquia anterior em dois animais do G1. Edema de córnea estava presente na área perilesional, em todos os grupos. Opacidade do cristalino foi visibilizada no G1, G2 e G3. A membrana no G2 e G3 atuou como barreira ao extravasamento do humor aquoso mantendoa superfície seca para a aplicação do adesivo, impediu o seu contato com as estruturas intraoculares e, associada ao adesivo, acelerou a reparação da lesão. A técnica cirúrgica utilizando a membrana amniótica associada ao n-butil 2-cianoacrilato foi de mais fácil execução e mais eficiente na reparação de lesões de córneas perfuradas do que as técnicas utilizando somente o adesivo ou somente a membrana amniótica suturada à córnea.
