A estratégia de busca foi aplicada à base de dados PubMed. Os artigos foram selecionados se abordassem as alterações microscópicas do tecido ósseo maxilofacial, observassem os fatores de risco envolvidos no processo e tivessem como foco os efeitos diretos e indiretos dos tratamentos radioterápicos de cabeça e pescoço. A eliminação de artigos foi realizada em duas etapas: leitura de título e resumo, e leitura integral dos artigos. Foram encontrados 847 artigos na base de dados, entre eles, 331 artigos foram selecionados para a avaliação, pelos critérios de inclusão e exclusão. Trinta e cinco trabalhos foram lidos na íntegra e 12 incluídos na revisão. A revisão dos artigos permitiu considerar que existe alteração celular promovida pela radiação ionizante, como a morte de osteócitos presentes na matriz e diminuição no número de osteoblastos e osteoclastos, que são responsáveis pela remodelação óssea. Além disso, os fatores encontrados na base de dados que podem aumentar a incidência da osteorradionecrose foram a técnica radioterápica usada, a dose de radiação prescrita, o uso do tabaco e procedimentos cirúrgicos pós-tratamento. Após o estudo, foi possível afirmar que existe mudança na conformação celular e morfológica do tecido ósseo irradiado, como um todo, com morte de osteoblastos, osteoclastos e osteócitos. The search strategy was applied to the PubMed database. The articles were selected if they dealt with the microscopic alterations of the maxillofacial bone tissue, observed the risk factors involved in the process and focused on the direct and indirect effects of radiotherapy on the head and neck. The elimination of articles was carried out in two stages: reading titles and abstract of the texts and reading the full articles. A total of 847 articles were found in the database, among them, 331 articles were selected for evaluation by inclusion and exclusion criteria. Thirty-five papers were read full articles and 12 were included in the review. The review of the articles allowed to consider that there is a cellular alteration promoted by ionizing radiation, such as the death of osteocytes present in the matrix and a decrease in the number of osteoblasts and osteoclasts, which are responsible for the bone remodeling. In addition, the factors found in the database that may increase the incidence of osteoradionecrosis were the radiotherapy technique used, the prescribed dose of radiation, the use of tobacco, and post-treatment surgical procedures. After the study, it was possible to affirm that there is a change in the cellular and morphological conformation of the irradiated bone tissue as a whole, with death of osteoblasts, osteoclasts osteocytes.
