Resumo: The tooth sensitivity after tooth whitening results from the ability of the bleaching agents to permeate the enamel, the dentinal tubes and reach the pulp promoting a reversible pulpitis. This painful response has been related to the concentration, time and frequency of application of the bleaching agent. Considering that 15% of patients who start tooth whitening do not continue treatment depending on the sensitivity, the objective of the present case report was to investigate the effectiveness of a home whitening technique with progressive onset regarding the presence of tooth sensitivity and the whitening effect. Homemade bleaching was performed with an individualized silicone tray and 10% carbamide peroxide with a progressive onset technique, in which the bleaching agent was used for ½ hour on the 1st day, 1 hour on the 2nd day, 2 hours on the 3rd day, 3 hours on the 4th day, 4 hours on the 5th day and 8 hours from the 6th to the 28th day of treatment. Sensitivity was measured using an objective questionnaire and tooth staining was assessed using traditional Vita and 3D Vita dental color scales. As a result, the patient had no sensitivity and the whitening effect was satisfactory. It is concluded that the home whitening technique with progressive onset is an effective strategy as it does not induce tooth sensitivity and provides a satisfactory whitening effect.

Esta resposta dolorosa tem sido relacionada a concentração, ao tempo e a frequência de aplicação do agente clareador. Considerando que 15% dos pacientes que iniciam o clareamento dentário não dão continuidade ao tratamento em função da sensibilidade, o objetivo do presente relato de caso clínico foi investigar a eficácia de uma técnica de clareamento caseiro com início progressivo quanto a presença de sensibilidade dentária e o efeito clareador. O clareamento caseiro foi realizado com moldeira de silicone individualizada e peróxido de carbamida 10% com técnica de início progressivo, em que o agente clareador foi utilizado por ½ hora no 1º dia, 1 hora no 2º dia, 2 horas no 3º dia, 3 horas no 4º dia, 4 horas no 5º dia e 8 horas do 6º ao 28º dia de tratamento. A sensibilidade foi mensurada por meio de um questionário objetivo e a coloração dentária foi avaliada com escalas de cor dentária Vita tradicional e Vita 3D. Como resultado, o paciente não apresentou sensibilidade e o efeito clareador foi satisfatório.
