Carcinoma incidental de vesícula biliar: um relato de caso

Resumo: Gallbladder cancer is a rare malignant neoplasm with a silent and aggressive progression, usually presenting a late diagnosis and with unfavorable prognosis. When the condition is incidentally found at cholecystectomy, it is called incidental gallbladder carcinoma, which is potentially curable and presents better diagnosis. The main goal of this article is to describe a case of a patient who had been diagnosed with incidental gallbladder carcinoma at cholecystectomy, paying attention to the importance of early monitoring of risk factors and carrying out a prophylactic cholecystectomy. In this case report: Female patient, 67 years old, with complains of epigastric pain and postprandial fullness five years prior to the first appointment. Six months after that, the condition got worse and irradiated to the right hypochondrium. An ultrasound revealed cholecystolithiasis and indicated videolaparoscopic cholecystectomy. The anatomopathological evidenced papillary-tubular neoplasm intracholecystic, with Tis staging tumor (In situ tumor). The surgery was curative, therefore reoperation was not recommended. This study emphasizes the importance of identifying the main risk factors for gallbladder cancer and shows the potential diagnostic and curative of cholecystectomy with biopsy when performed early. Through symptoms related to benign gallbladder disease and the association of predisposing factors to carcinoma, you can increase the chances of identifying this rare pathology. Further studies are needed on screening for early detection of gallbladder carcinoma, enabling better recognition and therapy for this challenging diagnosis neoplasm.

Quando a doença é encontrada incidentalmente, após realização de colecistectomia, denomina-se carcinoma incidental da vesícula biliar, o qual é potencialmente curável e de melhor prognóstico. O objetivo deste artigo é descrever um relato de caso de paciente que apresentou carcinoma incidental de vesícula biliar após colecistectomia, atentando para a importância do monitoramento precoce dos fatores de risco e realização de colecistostomia profilática. No relato de caso, tem-se uma paciente do sexo feminino, 67 anos, com queixa de dor epigástrica e plenitude pós-prandial há 5 anos. Referiu piora do quadro há 6 meses, com irradiação da dor para hipocôndrio direito. A ultrassonografia revelou colecistolitíase, sendo indicada colecistectomia videolaparoscópica. A análise anátomo-patológica evidenciou neoplasia papilífera-tubular intracolecística, com tumor em estadiamento Tis (Tumor in situ). A cirurgia foi curativa, não sendo recomendada a reoperação. Este estudo enfatiza a importância da identificação dos principais fatores de risco para a neoplasia de vesícula biliar e evidencia o potencial diagnóstico e curativo da colecistectomia com biópsia quando realizada precocemente. Através da sintomatologia referente à doença benigna da vesícula e da associação de fatores predisponentes ao carcinoma, pode-se aumentar as chances da identificação desta patologia rara.
