Cirurgia paraendodôntica: Um procedimento opcional para solução de patologias periapicais persistentes - Relato de caso

Resumo: The present study aims to report the resolution of a persistent chronic apical lesion in two anterior superior teeth through paraendodontic surgery. Female patient, 50 years old, came to the dental office with pain in the region of teeth 21 and 22. The clinical examination showed a gingival swelling located at the apex of tooth 22. She reported that she had just redone all the crowns superior anterior units and that due to the difficulty in removing the intraradicular nuclei, retreatment at that moment was not performed. Clinical examinations showed that the fixed unit prostheses were well adapted, however swelling in the apical region was evident. The radiographic examination showed persistent chronic apical lesions in teeth 21 and 22. The treatment plan of choice was the paraendodontic surgery in which an apicectomy would be performed, with retro-preparation and retro-obturation in the two dental elements and placement of bone graft and membrane. After seven days, the suture was removed and the patient's postoperative follow-up was performed. Periapical surgery can be considered an alternative to conventional retreatment in cases of persistent apical injury, in which there are factors such as difficulty in performing conventional retreatment, but also aesthetic and emotional factors to the patient. Whenever surgery is performed following correct techniques and appropriate materials, the prognosis is favorable in relation to bone repair and the maintenance and function of treated teeth.

Paciente de sexo feminino, 50 anos de idade, compareceu ao consultório odontológico com dor na região dos dentes 21 e 22. Ao exame clínico observou-se uma tumefação gengival localizada no ápice do dente 22. Ela relatou que tinha acabado de refazer todas as coroas unitárias anteriores superiores e que pela dificuldade de remoção dos núcleos intrarradiculares o retratamento naquele momento não foi realizado. Os exames clínicos mostraram que as próteses fixas unitárias estavam bem adaptadas, no entanto era evidente a tumefação em região apical. No exame radiográfico constatou-se lesões apicais crônicas persistentes nos dentes 21 e 22. O plano de tratamento de escolha foi a cirurgia paraendodôntica no qual seria realizado apicectomia, com retro- preparo e retro- obturação nos dois elementos dentários e colocação de enxerto ósseo e membrana. Após sete dias foi realizada a remoção de sutura e acompanhamento do pós-operatório da paciente. A cirurgia periapical pode ser considerada uma alternativa ao retratamento convencional em casos de lesão apical persistente, no qual existem fatores como dificuldade de realizar retratamento convencional , mas também fatores estéticos e emocionais ao paciente.
