The impact of overweight on clinical complications caused by COVID-19: A systematic review

Resumo: | The objective was to evaluate and show the main impacts of excessive corporal weight, related to the complexities and clinical outcomes, caused by SARS-CoV-2. This work is based in a systematic review of published works and driven by the question: “Which are the main questions about obesity as a risk factor related to the clinical complications of COVID and severe respiratory diseases?”, the researches were realized based on MEDLINE data, through the keywords available in DeCS/MeSH: Coronavírus/Coronavirus, Obesidade/ Obesity and SARS-Cov-2/Betacoronavirus, and then through pre-fixed criteria, were selected 13 articles to make part of the same research. The severity of the infection caused by SARS-CoV-2 increased with the CMI, in patients with overweight, the needs for intensive therapy and mechanical ventilation were about 50%. The overweight can act as a deregulator, harming the lymphoids and myeloides answers, which will disrupt the immunological system and extend the infections, contributing to a higher proliferation of viral infections, as occurs in the COVID-19 pandemics, besides, those individuals usually have some pre-existing metabolic and endocrine disturbances which can contribute to the use of the immune system, inflammatory and viral pathogens. The obesity and overweight are directly related with the intensification of patients symptoms with COVID-19, trough disturbances in the immune system, lack of respiratory capability and complacency of the respiratory system, and by the presence of associated diseases to that pathology. |

Estudio de revisión sistemática de literatura, guiado por la pregunta: "Cuáles son las notas principales sobre la obesidad como factor de riesgo asociado con complicaciones clínicas de COVID y enfermedades respiratorias graves?", las búsquedas se llevaron a cabo en la base de datos MEDLINE mediante la asociación de los siguientes descriptores disponibles en DeCS / MeSH: Coronavirus / Coronavirus, Obesity / Obesity and SARS-CoV-2 / Betacoronavirus, y se seleccionaron 13 criterios de inclusión preestablecidos artículos para ser parte de la investigación. Se observó que la gravedad de la infección por SARS-CoV-2 aumentaba con el IMC, la frecuencia de pacientes obesos en cuidados intensivos y que necesitaban ventilación mecánica era de alrededor del 50%. El exceso de peso puede actuar desregulando las respuestas linfoides y mieloides, lo que alterará el sistema inmune y extenderá las respuestas inflamatorias, contribuyendo a una mayor proliferación de infecciones virales, como ocurre en la pandemia de COVID-19, además, estos individuos generalmente tienen Trastornos metabólicos y endocrinos preexistentes que pueden contribuir al aumento de las respuestas inmunes e inflamatorias y la consiguiente propagación de patógenos virales. La situación de obesidad y sobrepeso afecta directamente la agravación y el aumento de las complicaciones de los síntomas de los pacientes con COVID-19, a través de trastornos en el sistema inmune, disminución del volumen de reserva respiratoria, capacidad funcional y complacencia del sistema respiratorio, y la presencia de enfermedades asociadas con esta patologia.O objetivo foi avaliar e demonstrar os principais impactos do excesso de peso nas complicações e desfechos clínicos ocasionados pelo SARS-CoV-2. Estudo de revisão sistemática da literatura, norteada pela pergunta: “Quais os principais apontamentos acerca da obesidade como um fator de risco associado às complicações clínicas do COVID e doenças respiratórias graves?”, as pesquisas foram realizadas na base de dados MEDLINE através da associação dos seguintes descritores disponíveis no DeCS/MeSH: Coronavírus/Coronavirus, Obesidade/Obesity e SARS-CoV-2/Betacoronavirus, e a partir dos critérios de inclusão pré-estabelecidos foram selecionados 13 artigos para fazerem parte da pesquisa. Observou-se que a gravidade da infecção por SARS-CoV-2 aumentou com o IMC, a frequência de pacientes obesos em terapia intensiva e com necessidade de ventilação mecânica era em torno de 50%. O excesso de peso pode atuar desregulando as respostas linfoides e mieloides, as quais irão alterar o sistema imunológico e estender respostas inflamatórias, contribuindo para a maior proliferação de infecções virais, como ocorre na pandemia do COVID-19, além disso, esses indivíduos geralmente possuem distúrbios metabólicos e endócrinos preexistentes que podem contribuir para o aumento das respostas imunes, inflamatórias e consequente disseminação dos patógenos virais.
