Prevalence of hypertension and selection of local anesthetics in patients seen at a school dental clinic

Resumo: Arterial hypertension (AH) is a chronic disease, determined by pressure levels above 140/90 mm/Hg. Data from the Ministry of Health (MS) reveal that more than 30 million Brazilians suffer from diagnosed AH and it is estimated that more than half are unaware of the presence of the disease or do not undergo treatment. The present study aimed to assess the prevalence of hypertensive patients treated by dental clinics at UNIPAR, as well as to review the literature on oral alterations caused by the use of antihypertensive drugs and the use of local anesthetics (LA) with or without vasoconstrictor (VS). Cross-sectional/prevalence study, where the research was carried out at the dental clinics of UNIPAR campus Cascavel, where volunteers were instructed to complete a structured form referring to their systemic condition, novice and hygiene habits, the data obtained were tabulated and submitted to statistical analysis. Of the 854 volunteers, 57.25% (n = 489) had no systemic changes, 42.74% (n = 365) had systemic changes, of which 38.22% (n = 133) individuals had hypertension. It can be concluded that the dental surgeon (CD) within this context has an important role in the diagnosis of patients with this condition, since the vast majority are unaware of having the disease. The prevalence of hypertensive patients seeking dental care is relatively high. In addition, it is important to control the patient's anxiety, whether orally or inhaled, have good anesthetic technique and care in the choice of medications that will be used in the postoperative period.

Datos del Ministerio de Salud (MS) revelan que más de 30 millones de brasileños padecen HA diagnosticada y se estima que más de la mitad desconocen la presencia de la enfermedad o no reciben tratamiento. El presente estudio tuvo como objetivo evaluar la prevalencia de pacientes hipertensos atendidos en las clínicas dentales de UNIPAR, así como revisar la literatura sobre las alteraciones bucales provocadas por el uso de antihipertensivos y el uso de anestésicos locales (AL) con o sin vasoconstrictor ( VS). Estudio transversal / prevalencia, donde la investigación se llevó a cabo en las clínicas dentales de UNIPAR campus Cascavel, donde se instruyó a los voluntarios a completar un formulario estructurado referente a su condición sistémica, novicio y hábitos de higiene, los datos obtenidos fueron tabulados y sometidos a análisis estadístico . De los 854 voluntarios, el 57,25% (n = 489) no presentaron cambios sistémicos, el 42,74% (n = 365) presentaron cambios sistémicos, de los cuales el 38,22% (n = 133) individuos tenían hipertensión. Se puede concluir que el cirujano dentista (EC) en este contexto tiene un papel importante en el diagnóstico de los pacientes con esta condición, ya que la gran mayoría desconoce que padece la enfermedad. La prevalencia de pacientes hipertensos que buscan atención odontológica es relativamente alta. Además, es importante controlar la ansiedad del paciente, ya sea por vía oral o inhalada, tener una buena técnica anestésica y cuidado en la elección de los medicamentos que se utilizarán en el postoperatorio.A hipertensão arterial (HA) é uma doença crônica, determinada por níveis pressóricos acima de 140/90 mm/Hg.  Dados do Ministério da Saúde (MS) revelam que mais de 30 milhões de brasileiros sofrem de HA diagnosticada e estima-se que mais da metade desconhecem a presença da doença ou não fazem tratamento. O presente trabalho se objetivou em avaliar a prevalência de pacientes hipertensos atendidos pelas clínicas odontológicas da UNIPAR, bem como revisar a literatura a respeito de alterações bucais ocasionadas pelo uso de anti-hipertensivos e o uso de anestésicos locais (AL) com ou sem vasoconstritor (VS). Estudo transversal/prevalência, onde a pesquisa foi realiza nas clínicas odontológicas da UNIPAR campus Cascavel, onde os voluntários foram orientados a preencherem um formulário estruturado referente à sua condição sistêmica, hábitos noviços e de higiene, os dados obtidos foram tabulados e submetidos à análise estatística. Dos 854 voluntários, 57,25% (n=489) não apresentaram alterações sistêmicas, 42,74% (n=365) apresentaram alterações sistêmicas, dos quais 38,22% (n=133) indivíduos possuíam hipertensão. Pode se concluir que o cirurgião dentista (CD) dentro deste contexto tem um importante papel no diagnóstico de pacientes com essa condição, visto que em sua grande maioria desconhecem serem portadores da doença. É relativamente alta a prevalência de hipertensos que procuram atendimento odontológico. Além disso, é importante controlar a ansiedade do paciente, seja de forma oral ou inalatória, ter boa técnica anestésica e cuidado na escolha das medicações que serão utilizadas no pós-operatório.
