Family evaluation of an elderly with endemic goiter in primary health care

Resumo: The family is the focus of attention of the Family Health Strategy (FHS). In this sense, using tools to approach the family can help the professional to get to know it, being able to analyze it in its context and more deeply the dynamics of family relationships and understand how it influences the health-disease-care process. For this purpose, a case study was carried out, with the objective of evaluating the functionality of a family in the district of jaibaras Sobral-CE, using the  APGAR tools for  family, genogram and ecomap. The index case was female, 60 years old, retired, married, diagnosed with systemic arterial hypertension and endemic goiter. The analysis showed a strong link between most family members and the index case, and a conflicting relationship between this and a son, who has bipolar disorder. In addition, the external structure showed us good monitoring of the FHS in relation to the health of all family members. Based on the application of the APGAR Familiar questionnaire, the family can be classified as having good functionality, since it obtained a score of eight. Thus, the findings point to the importance of developing strategies that involve health professionals in the use of assessment tools with families, encouraging them to adopt proximity to the context of the home and their social and health network throughout life, allowing effective and humanizing actions.

En este  sentido, utilizar herramientas de abordaje a la familia puede auxiliar al profesional a conocerla, pudiendo analizarla en su contexto y de una forma más profunda la dinámica de las relaciones familiares y entender como influye en el proceso salud-enfermedad-cuidado. Para ello se realizó un estudio de caso, con el objetivo de evaluar la funcionalidad de una familia del distrito de Jaibaras Sobral-Ceará por medio de las herramientas APGAR familiar, genograma y ecomapa. El caso índice era de sexo femenino, 60 años, jubilada, casada, con diagnóstico de hipertensión arterial sistémica y bocio endémico. El análisis evidenció fuerte vínculo entre la mayor parte de los miembros de la familia con el caso índice, y una relación conflictiva de esta con un hijo, que padece trastorno bipolar. Además, la estructura externa nos mostró un buen seguimiento de la ESF en relación a la salud de todos los miembros de la familia. Con base en la aplicación del cuestionario de APGAR Familiar, se puede clasificar a la familia con buena funcionalidad, ya que obtuvo resultado ocho. Así, los resultados indican la importancia del desarrollo de estrategias que impliquen a los profesionales de salud en la utilización de las herramientas de evaluación con familias, estimulándolos a aproximarse al contexto del hogar y de la red social y sanitaria de estas a lo largo de la vida, permitiendo así acciones efectivas y humanizadoras.A família é o foco da atenção da Estratégia Saúde da Família (ESF). Nesse sentindo, utilizar ferramentas de abordagem à família pode auxiliar o profissional a conhecê-la, podendo analisa-la em seu contexto e de forma mais profunda a dinâmica das relações familiares e entender como ela influencia no processo saúde-doença-cuidado. Para tanto, realizou-se um estudo de caso, com o objetivo de avaliar a funcionalidade de uma família do distrito de jaibaras Sobral-CE, por meio das ferramentas APGAR familiar, genograma e ecomapa. O caso índice era do sexo feminino, 60 anos, aposentada, casada, com diagnóstico de hipertensão arterial sistêmica e bócio endêmico. A análise evidenciou forte vínculo entre a maior parte dos membros da família com o caso índice, e uma relação conflituosa desta com um filho, que tem transtorno bipolar. Além disso, a estrutura externa nos mostrou bom acompanhamento da ESF em relação à saúde de todos os membros da família. Com base na aplicação do questionário de APGAR Familiar, pode-se classificar a família com boa funcionalidade, uma vez que obteve resultado oito.
