Therapeutic itinerary of hematological patients: the importance of early diagnosis

Resumo: Objective: to identify the trajectory of patients with acute leukemia and severe aplastic anemia before the specialized care seeking care at the reference center. Methods: cross-sectional, observational, quantitative and analytical research. The research was carried out in a university hospital in the south of Brazil, in two services that care for onco-hematological patients. In the 18-month period, 237 patients were treated, according to the inclusion criteria: adults over 18 years of age, with prior treatment for education and with diagnostic confirmation, being eligible for the study, 31 patients with acute leukemia and 19 with severe aplastic anemia (n=50). Results: the median time of the search interval for help was 7 (0-158) days; diagnostic interval of 14 (0-390) days; and pre-treatment interval 2 (71-783) days. In search of care, in the first ten days after the onset of two symptoms, it occurred in 16 (61.5%) patients with acute leukemia and in 7 (41.2%) patients with severe aplastic anemia (p≥0.05). The time interval between the diagnosis, proximity to the specialized center and type of work will show statistical significance. Conclusion: the knowledge of two factors that led to the non-diagnostic delay of these actions, contributed to the understanding of health care, contributing to improve hematological assistance and the formulation of public policies, including effective strategies to improve access to specialized services.

Métodos: investigación transversal, observacional, cuantitativa y analítica. La investigación se llevó a cabo en un hospital universitario en el sur de Brasil, en el servicio de atención a pacientes onco-hematológicos. En en período de 18 meses fueron atendidos 237 pacientes, según los criterios de inclusión: adultos mayores de 18 años, sin tratamiento previo para la enfermedad y con confirmación diagnóstica, siendo elegibles para el estudio, 31 pacientes con leucemia aguda y 19 con anemia aplásica severa (n=50). Resultados: la mediana del tiempo del intervalo de búsqueda de ayuda fue de 7 (0-158) días; intervalo diagnóstico de 14 (0-390) días; e intervalo de pretratamiento 2 (71-783) días. La búsqueda de atención, en los primeros diez días después del inicio de los síntomas, ocurrió en 16 (61,5%) pacientes con leucemia aguda y por 7 (41,2%) pacientes con anemia aplásica severa (p≥0,05). El intervalo de tiempo entre el diagnóstico, la proximidad al centro especializado y el tipo de enfermedad mostró significacion estadística. Conclusión: el conocimiento de los factores que conducen al retraso en el diagnóstico de estas enfermedades ayuda a la comprensión de la atencion de la salud, contribuyendo para mejorar la atención hematológica y la formacion de políticas públicas, incluyendo estrategias efectivas para mejorar el acceso a los servicios especializados.Objetivo: identificar a trajetória de pacientes com leucemias agudas e anemia aplástica severa na busca por atendimento até um centro de referência. O intervalo de tempo entre o diagnóstico, proximidade ao centro especializado e tipo da doença demostraram significância estatística.
