Analysis of the presence of pressure injury in hospitalized patients and the main associated comorbities

Resumo: The present study aimed to analyze the presence of Pressure Injury (PI) in hospitalized patients, observing the main associated comorbidities. This is a retrospective document analysis, with an analytical observational profile, of a quantitative and transversal character. The research was carried out in a highly complex hospital in the public network, located in the state of Paraíba and was carried out through a careful analysis of 884 medical records of patients admitted from January to December 2019, including those who had at least an PI during your hospital stay. Information such as sociodemographic profile, age, sex, clinical history of the patient and associated diseases were evaluated, in addition to the Braden scale score to identify the risk of developing the lesion. The sample consisted of 85 patients, most of whom (83.5%) were over 65 years of age, 47 (55.29%) were female and 38 (44.71%) were male. An incidence of 11.5% of the presence of PI was observed in patients, characterized as a number to be taken into account, since it is a complication classified as preventable in most cases, and it is also known underreporting as it is considered a medical malpractice problem. Diabetes Mellitus (DM) and Systemic Arterial Hypertension (SAH) were identified as the main associated comorbidities, and of the patients who developed PI, 10 (11.8%) had DM, 21 (24.7%) had SAH and 33 (38.8%) had both diseases, with implications for wound healing and worsening. Therefore, it is relevant to emphasize the importance of systematic monitoring of the patient, in order to verify the existing comorbidities and the risks of the occurrence of the injury, in order to maximize the preventive care, providing a better hospital stay for the patient and avoiding high treatment costs.

Este es un análisis documental retrospectivo con un perfil analítico observacional, de carácter cuantitativo y transversal. La investigación se llevó a cabo en un hospital público altamente complejo, ubicado en el estado de Paraíba, y se realizó mediante un análisis cuidadoso de 884 registros médicos de pacientes ingresados de enero a diciembre de 2019, incluidos aquellos que tenían al menos un LP durante su estadía en el hospital. Se evaluó información como el perfil sociodemográfico, la edad, el sexo, el historial clínico del paciente y las enfermedades asociadas, además de la puntuación en la escala de Braden para identificar el riesgo de desarrollar la lesión. La muestra consistió en 85 pacientes, la mayoría de los cuales (83.5%) tenían más de 65 años, 47 (55.29%) eran mujeres y 38 (44.71%) eran hombres. Se observó una incidencia del 11,5% de la presencia de LP en pacientes, caracterizada como un número a tener en cuenta, ya que es una complicación clasificada como prevenible en la mayoría de los casos, y también se conoce subregistro ya que se considera un problema de negligencia médica. La diabetes mellitus (DM) y la hipertensión arterial sistémica (HSA) se identificaron como las principales comorbilidades asociadas, y de los pacientes que desarrollaron LP, 10 (11.8%) tenían DM, 21 (24.7%) tenían HSA y 33 (38.8%) tenían ambas enfermedades, con implicaciones para la cicatrización y empeoramiento de heridas. Por lo tanto, es importante enfatizar la importancia del monitoreo sistemático del paciente, a fin de verificar las comorbilidades existentes y los riesgos de ocurrencia de la lesión, para maximizar la atención preventiva, proporcionar una mejor estadía en el hospital para el paciente y evitar altos costos de tratamiento.O presente estudo objetivou analisar a presença de Lesão por Pressão (LP) em pacientes hospitalizados, observando as principais comorbidades associadas. A pesquisa foi realizada em um hospital de alta complexidade da rede pública, situado no estado da Paraíba e se deu por análise criteriosa de 884 prontuários médicos dos pacientes internados nos meses de janeiro a dezembro de 2019, sendo incluídos no estudo aqueles que possuíssem pelo menos uma LP durante sua estadia hospitalar. Foram avaliadas informações como perfil sociodemográfico, idade, sexo, histórico clínico do paciente e enfermidades associadas, além de observada a pontuação na escala de Braden para identificar o risco de desenvolvimento da lesão. A amostra foi composta por 85 pacientes, que apresentaram em sua maioria (83,5%) mais de 65 anos de idade, sendo 47 (55,29%) do sexo feminino e 38 (44,71%) do sexo masculino. Foi observada uma incidência de 11,5% de presença de LP nos pacientes, caracterizando-se como um número ao qual deve-se atentar, visto que se trata de uma complicação classificada como evitável na maioria dos casos, e sabendo-se ainda da existência de subnotificação por ser considerado um problema de negligência médica. A Diabetes Mellitus (DM) e a Hipertensão Arterial Sistêmica (HAS) foram apontadas como as principais comorbidades associadas, sendo que dos pacientes que desenvolveram LP, 10 (11,8%) possuíam DM, 21 (24,7%) possuíam HAS e 33 (38,8%) possuíam as duas doenças, acarretando implicações na cicatrização e agravamento da lesão.
