Successful endodontic treatment on an upper lateral incisor with non-perforating inflammatory internal root resorption: case report

Resumo: Internal tooth resorption is usually asymptomatic and is almost always diagnosed on routine radiographic examinations. The objective of this study is to present a clinical case report of a tooth affected by internal non-perforating inflammatory root resorption, in order to disseminate information that can be used to guide professionals in these cases, helping with therapeutic management. A 53-year-old female patient was referred to the specialization course at Faculdade CIODONTO after performing a routine periapical radiographic examination, which showed non-perforating internal resorption at the level of the radicular middle third of the tooth 12. Radiographically it was verified that the contour of the pulp boundaries undergoes a relatively symmetrical expansion with a ballooning aspect, with regular and rounded contours. After coronary opening, bleeding compatible with the vital pulp was observed, initially irrigating with lime water. Biomechanical preparation using the crown-apex technique without pressure was performed and Callen was selected as an intracanal medication for 30 days and L&C paste for 90 days. After this period, root canal filling was performed using the Tagger hybrid thermoplastic technique and calcium hydroxide-based endodontic cement. Proservation was performed after 1 year and the success of endodontic therapy was confirmed. Thus, according to the protocol indicated in the literature, effective clinical resolution of teeth affected by internal non-perforating resorption is allowed, proving to be an excellent alternative for conservative treatment.

El objetivo de este estudio es presentar un caso clínico de diente afectado por reabsorción radicular inflamatoria interna no perforante, con el fin de difundir información que sirva de guía a los profesionales en estos casos, ayudando en el manejo terapéutico. Una paciente de 53 años fue remitida al curso de especialización de la Faculdade CIODONTO luego de realizarle un examen radiográfico periapical de rutina, el cual mostró reabsorción interna no perforante a nivel del tercio medio radicular del diente 12. Radiográficamente se verificó que el contorno de los límites pulpares experimenta una expansión relativamente simétrica con un aspecto abombado, con contornos regulares y redondeados. Tras la apertura coronaria se observó sangrado compatible con la pulpa vital, regando inicialmente con agua de cal. Se realizó preparación biomecánica mediante la técnica corona-ápice sin presión y se seleccionó a Callen para medicación intracanal durante 30 días y pasta L&C durante 90 días. Después de este período, el relleno del conducto radicular se realizó mediante la técnica termoplástica híbrida de Tagger y cemento endodóntico a base de hidróxido de calcio. La proservación se realizó después de 1 año y se confirmó el éxito de la terapia endodóntica. Así, de acuerdo con el protocolo indicado en la literatura, se permite la resolución clínica efectiva de los dientes afectados por reabsorción interna no perforante, demostrando ser una excelente alternativa para el tratamiento conservador.As reabsorções dentárias internas são geralmente assintomáticas e quase sempre diagnosticadas em exames radiográficos de rotina. O objetivo deste estudo é apresentar um relato de caso clínico de um dente acometido por reabsorção radicular interna inflamatória não perfurante, com intuito de divulgar informações que sirvam de subsídios para orientar os profissionais frente a esses casos, auxiliando na conduta terapêutica. Paciente do sexo feminino, 53 anos de idade, foi encaminhada ao curso de especialização da Faculdade CIODONTO após realização de um exame radiográfico periapical de rotina, o qual mostrou reabsorção interna não perfurante à nível de terço médio radicular do dente 12. Radiograficamente foi verificado que o contorno dos limites pulpares sofre uma expansão relativamente simétrica de aspecto balonizante, contornos regulares e arredondados. Após a abertura coronária, foi observado sangramento compatível à polpa vital, irrigando-se, inicialmente, com água de Cal. O preparo biomecânico pela técnica coroa-ápice sem pressão foi realizado e como medicação intracanal foi selecionado o Callen por 30 dias e pasta L&C por 90 dias. Após esse período, foi realizada a obturação dos canais radiculares pela técnica termoplástica híbrida de Tagger e cimento endodôntico à base de hidróxido de cálcio. A proservação foi realizada após 1 ano e o sucesso da terapia endodôntica foi constatado.
