Overweight and childhood obesity: the influence of social determinants of health in a municipality in the Bahia region

Resumo: This study aimed to analyze the relationship between the social determinants of health with the occurrence of overweight and childhood obesity in schools in the town of Santo Amaro, in the Bahian Recôncavo. It's an epidemiological, descriptive and quantitative cross-sectional study involving 82 schoolchildren born in 2012, enrolled in public and private schools. Sociodemographic data and eating habits were obtained through interviews with the mothers of schoolchildren at home. Birth weight and gestacional age were obtained through consultation with the child's health card. Anthropometric data were collected from the schoolchildren and the BMI was further calculated classified according to the Z score curves recommend by the World Health Organization. A high prevalence of overweight/childhood obesity was identified (40,24%). The male sex (24.39%) compared to the female sex (15.85%), obtained a higher percentage (p<0.2255). Regarding the sociodemographic area, a high prevalence was observed in town (31,57%), with 32% living in rural areas and 68% in urban areas. Regarding inadequate eating habits, 46.72% of schoolchildren who started to consume food with a high carbohydrate rate at an early age had a higher prevalence (p<0.02479). A strong associations was verified in families with wage income below a minimum wage (p<0.0001). The analysis of the Social Determinants of Health showed to be fundamental to understand the epidemiological situation, being evidenced high rate of overweight/childhood obesity.

Se trata de un estudio epidemiológico, de carácter descriptivo, cuantitativo de tipo transversal, en el que participan 82 escolares nacidos en 2012, matriculados en escuelas públicas y privadas. Datos sociodemográficos y hábitos alimentarios fueron obtenidos por medio de entrevistas con las madres de los escolares en los domicílios. El peso al nacer y la edad gestacional fueron obtenidos por medio de consulta a la libreta de salud del niño. Se recogieron datos antropométricos de los escolares en las escuelas y se calculó el IMC, posteriormente, clasificados según curvas de escores-z preconizadas por la Organización Mundial de la Salud. Se identificó una alta prevalencia para sobrepeso/obesidad infantil (40,24%). El sexo masculino (24,39%) comparado al sexo femenino (15,85%), obtuvo mayor porcentaje (p< 0,0255). Con relación a área sociodemográfica, se observó elevada prevalencia en el municipio (31,57%), siendo 32% moradores de zona rural y 68% de zona urbana. En cuanto a los hábitos alimentarios inadecuados, el 46,72% de los escolares que empezaron a consumir desde temprano alimentos con alta tasa de carbohidratos presentaron mayor prevalencia (P<0,02479). Se comprobó una fuerte asociación en familias con ingresos salariales inferiores a un salario mínimo (P<0,0001). El análisis de los determinantes sociales de la salud ha demostrado ser fundamental para evaluar la situación epidemiológica, siendo evidenciado alta tasa de sobrepeso/obesidad infantil.Objetivou-se analisar a relação entre os determinantes sociais da saúde com a ocorrência do sobrepeso e da obesidade infantil em escolares no município de Santo Amaro, no Recôncavo Baiano. Com relação a área sociodemográfica, observou-se elevada prevalência no município (31,57%), sendo 32% moradores da zona rural e 68% da zona urbana. No tocante aos hábitos alimentares inadequados, 46,72% dos escolares que passaram a consumir desde cedo alimentos com elevada taxa de carboidrato apresentaram maior prevalência (p<0,02479). Foi verificada uma forte associação em famílias com renda salarial inferior a um salário mínimo (p<0,0001).
