Epidemiological and obstetric profile of women with suspected or confirmed SARS-CoV-2 infection in a public maternity hospital

Resumo: | Objective: to describe the epidemiological and obstetric profile of women and newborns of mothers with suspected or confirmed SARS-CoV-2 infection. Method: this is a cross-sectional, descriptive, retrospective study with data collected from medical records of women treated at a public maternity hospital in the city of Belo Horizonte in the respiratory isolation sector between March and December 2020, including suspected cases and confirmed for SARS-CoV-2. Result: 98 medical records of pregnant patients were analyzed. Regarding pregnant women, five were twin pregnancies and one was triplet, totaling 104 newborns in the study. Only 12 women (11.7%) had confirmed SARS-CoV-2 infection, among which the main associated comorbidity was Gestational diabetes Mellitus and cesarean section as the route of birth. There were no deaths among the newborns. Among the suspected cases that tested negative, the main associated comorbidity was hypertension in pregnancy, most evolved to normal delivery and four newborns evolved to death unrelated to COVID-19. Both groups had mild symptoms on admission; newborns received Apgar ≥8 at the first and fifth minutes. Conclusion: the description of the epidemiological and obstetric profile showed significant associations with the comorbidities diabetes mellitus, gestational hypertension, and increased the incidence of cesarean section in the group of positive patients. Neonatal outcomes did not show negative outcomes. The knowledge of these characteristics helps in the construction of care for the study population and the formulation of actions in the care service that qualify the assistance and bring resolution to clinical situations. |

Método: se trata de un estudio transversal, descriptivo, retrospectivo con datos recolectados de las historias clínicas de mujeres atendidas en una maternidad pública de la ciudad de Belo Horizonte en el sector de aislamiento respiratorio entre marzo y diciembre de 2020, incluyendo casos sospechosos y confirmados para SARS-CoV-2. Resultado: se analizaron 98 historias clínicas de pacientes embarazadas. En cuanto a las gestantes, cinco fueron embarazos gemelares y uno de trillizos, totalizando 104 recién nacidos en el estudio. Solo 12 mujeres (11.7%) tenían infección por SARS-CoV-2 confirmada, entre las cuales la principal comorbilidad asociada fue la Diabetes Mellitus Gestacional y la cesárea como vía de parto. No hubo muertes entre los recién nacidos. Entre los casos sospechosos que dieron negativo, la principal comorbilidad asociada fue hipertensión en el embarazo, la mayoría evolucionó a parto normal y cuatro recién nacidos evolucionaron a muerte no relacionada con COVID-19. Ambos grupos presentaron síntomas leves al ingreso; los recién nacidos recibieron Apgar ≥8 al primer y quinto minuto. Conclusión: la descripción del perfil epidemiológico y obstétrico mostró asociaciones significativas con comorbilidades: diabetes mellitus, hipertensión gestacional, además de aumentar la incidencia de cesárea en el grupo de pacientes positivas. Los resultados neonatales no mostraron resultados negativos. Resultado: foram analisados 98 prontuários de pacientes gestantes. Em relação às gestantes, cinco foram gestações de gemelar e uma de trigemelar, totalizando 104 recém-nascidos no estudo. Apenas 12 mulheres (11,7%) tiveram confirmação de infecção pelo SARS-CoV-2, dentre as quais a principal comorbidade associada era diabetes Mellitus Gestacional e a cesariana como via de nascimento. Não houve óbito dentre os recém-nascidos. Dentre os casos suspeitos que testaram negativo, a principal comorbidade associada foi a hipertensão na gravidez, a maioria evoluiu para parto normal e quatro recém-nascidos evoluíram para óbito não relacionado à COVID-19. Ambos os grupos tiveram sintomas leves na admissão, os recém-nascidos receberam Apgar ≥8 no primeiro e quinto minuto. Conclusão: a descrição do perfil epidemiológico e obstétrico apontou associações significativas com comorbidades: diabetes mellitus, hipertensão gestacional, além de aumentar a incidência de cesariana no grupo das pacientes positivas. Os resultados neonatais não apresentaram desfechos negativos.
