Association between prematurity and feeding difficulties in infancy: systematic review

Resumo: Objective: to perform a systematic review of the literature to analyze the hypothesis of association between prematurity and eating difficulties in childhood. Methods: the systematic review was conducted according to the PRISMA guidelines. The research was conducted by two independent researchers on PubMed and SciELO databases using the following combined keywords: “premature infants", “premature newborn”, “preterm”, “eating disorders”, “eating disorders in childhood”, “eating behavior”, “disease in premature infants”, “nutritional disorders of nursing infants", and its correlates in Portuguese. Articles published from 1996 to 2022 were included in our review. Results: of the 45 eligible articles, 21 were included in the review. 19 studies (90,5%) associated prematurity to a risk factor for the development of eating difficulties in childhood. The main eating difficulties were related to introduction of solid diet, oral motor difficulty, refusal/avoidance of food, coughing, vomiting, appetite and pleasure in eating reduced, disorganized or dysfunctional feeding, greater selectivity, food neophobia, and poorer feeding compared to children at term. Ten studies (47,6%) indicated that the lower the gestational age and the lower the birth weight, the higher the prevalence of eating difficulties. Conclusions: There is important evidence associating prematurity to specific eating difficulties in childhood. However, the methodological weaknesses of the studies included in our review decrease the quality of the evidence produced.

Se evaluaron artículos publicados entre 1996 y 2022. Resultados: de los 45 artículos elegibles, 19 fueron incluidos en la revisión. En 19 estudios (90,5%) hubo asociación entre la prematuridad y las dificultades de alimentación en la infancia. Las principales dificultades alimentarias encontradas se relacionaron con la introducción de alimentos sólidos, dificultad oromotora, rechazo/evitación de alimentos, tos, vómitos, disminución del apetito y del placer de comer, alimentación desorganizada o disfuncional, mayor selectividad, neofobia alimentaria y dieta más empobrecida en comparación con hijos a término. Diez estudios (47,6%) indicaron que a menor edad gestacional y menor peso al nacer, mayor prevalencia de dificultades de alimentación. Conclusión: existe evidencia importante que asocia la prematuridad con las dificultades de alimentación en la infancia, sin embargo, las debilidades metodológicas reducen la calidad de la evidencia producida.Objetivo: revisar sistematicamente a literatura para avaliar a hipótese de associação entre prematuridade e dificuldades alimentares na infância. Foram avaliados artigos publicados entre 1996 e 2022. Resultados: dos 45 artigos elegíveis, 21 foram incluídos na revisão. Em 19 estudos (90,5%) verificou-se a associação entre prematuridade e dificuldades alimentares na infância. As principais dificuldades alimentares encontradas foram relacionadas à introdução da alimentação sólida, dificuldade oromotora, recusa/evitação dos alimentos, tosse, vômitos, apetite e prazer em comer reduzidos, alimentação desorganizada ou disfuncional, maior seletividade, neofobia alimentar e alimentação mais empobrecida em comparação a crianças a termo. Dez estudos (47,6%) indicaram que quanto menor a idade gestacional e menor o peso ao nascer maior é a prevalência de dificuldades alimentares.
