Photoelastic analysis of narrow implants in atrophic maxilla: in vitro study

Resumo: | Rehabilitation in atrophic maxilla brings challenges due to the continuous and progressive alveolar resorption and alveolar pneumatization of the maxillary sinus, which makes rehabilitation with conventional size implants unfeasible. One of the alternatives for these cases is the use of narrow implants. Thus, the aim of the present study was to verify the viability of narrow implants in the total rehabilitation of the atrophic maxilla. The photoelastic analysis method was used, creating six groups: GC1 (4.1 mm x 10.0 mm), GC2 (4.1 mm x 12.0 mm), NC1 (3.3 mm x 10.0 mm), NC2 (3.3 mm x 12.0 mm) , SC1 (2.9 mm x 10.0 mm) and SC2 (2.9 mm x 12.0 mm) that were evaluated in the circular polariscope model PTH-A-01 (LPM/FME/UFU) with centric loading, left/right laterality and 5 N protrusive The models were tested, photographed and analyzed according to isometric fringe orders. The analysis of color patterns and location aided in the quantitative-qualitative interpretation and tensions of the subgroups tested arising from the forces on osseointegrated implants arranged in an arch, “all-on-four” rehabilitation type in the maxilla. The evaluated groups showed a positive interaction in relation to the increase in the diameter of the implants, with a decrease in the stress concentration for the region of the central implants. No difference was observed between the groups in the protrusive loading and, in relation to the distal loadings, the stresses in all groups ranged from low to high. Thus, in general, no significant differences were found in relation to narrow implants when compared to regular diameter implants. |

Una de las alternativas para estos casos es el uso de implantes estrechos. Así, el objetivo del presente estudio fue verificar la viabilidad de los implantes angostos en la rehabilitación total del maxilar atrófico. Se utilizó el método de análisis fotoelástico, creando seis grupos: GC1 (4,1 mm x 10,0 mm), GC2 (4,1 mm x 12,0 mm), NC1 (3,3 mm x 10,0 mm), NC2 (3,3 mm x 12,0 mm), SC1 (2,9 mm x 10,0 mm) y SC2 (2,9 mm x 12,0 mm) que fueron evaluados en el polariscopio circular modelo PTH-A-01 (LPM/FME/UFU) con carga céntrica, lateralidad izquierda/derecha y 5 N protrusivos Los modelos fueron probados, fotografiados y analizados según órdenes de franjas isométricas. El análisis de los patrones de color y ubicación ayudó en la interpretación cuantitativo-cualitativa y las tensiones de los subgrupos probados que surgen de las fuerzas sobre los implantes osteointegrados dispuestos en un arco, tipo de rehabilitación “todo sobre cuatro” en el maxilar. Los grupos evaluados mostraron una interacción positiva en relación al aumento del diámetro de los implantes, con una disminución en la concentración de estrés para la región de los implantes centrales. No se observaron diferencias entre los grupos en la carga protrusiva y, en relación con las cargas distales, las tensiones en todos los grupos variaron de bajas a altas. Utilizou-se do método da análise fotoelástica, confeccionando-se seis grupos: GC1 (4.1 mm x 10.0 mm), GC2 (4.1 mm x 12.0 mm), NC1 (3.3 mm x 10.0 mm), NC2 (3.3 mm x 12.0 mm), SC1 (2.9 mm x 10.0 mm) e SC2 (2.9 mm x 12.0 mm) que foram avaliados no polariscópio circular modelo PTH-A-01 (LPM/FME/UFU) com carga cêntrica, lateralidade esquerda/direita e protrusiva de 5 N. Os modelos foram ensaiados, fotografados e analisados de acordo com as ordens de franjas isométricas. A análise dos padrões de cores e localização auxiliou na interpretação quanti-qualitativa e das tensões dos subgrupos testados oriundos das forças sobre implantes osseointegrados dispostos em arco, tipo reabilitação “all-on-four” em maxila. Os grupos avaliados demonstraram interação positiva com relação ao aumento do diâmetro dos implantes, com diminuição da concentração de tensão para a região dos implantes centrais. Não foi observada diferença entre os grupos no carregamento protrusivo e, em relação aos carregamentos distais, esquerdo as tensões em todos os grupos variaram de baixa a alta.
