Resumo: Introduction: Menstrual bleeding occurs as a universal, self-limiting endometrial event that follows a drop in hormone levels (estrogen and progesterone suppression) in a normal ovulatory cycle. The mechanism is complex and orderly, so that dysregulation of the sequential molecular, cellular and vascular events involved can lead to a marked variety of menstrual disorders. Objective: to elucidate the main causes of abnormal uterine bleeding, as well as its etiologies. Methodology: this is a narrative literature review. The research was carried out through online access to the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO databases. Information Services. Results and Discussion: The term “Abnormal Uterine Bleeding” (AUB) includes dysfunctional bleeding (anovulatory or ovulatory) and bleeding from structural causes (fibroids, polyps, adenomyosis, adenomyomas, endometrial carcinoma, pregnancy complications). AUB may also be related to the use of hormonal contraceptives or hormone therapy. It can affect all age groups, from adolescence to post-menopause. The highest prevalence is recorded at the extremes of reproductive life, particularly in adolescence and perimenopause, periods that are characterized by a higher concentration of anovulatory or irregular cycles. Final considerations: A medical history should be directed towards identifying the nature of the bleeding, identifying possible structural or organic causes, impacting on quality of life and assessing the woman's expectations, especially regarding the need for contraception or desire for pregnancy.

La investigación se llevó a cabo a través del acceso en línea a la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y las bases de datos EBSCO. Servicios. Resultados y Discusión: El término “Sangrado Uterino Anormal” (SUA) incluye sangrado disfuncional (anovulatorio u ovulatorio) y sangrado por causas estructurales (fibromas, pólipos, adenomiosis, adenomiomas, carcinoma endometrial, complicaciones del embarazo). AUB también puede estar relacionado con el uso de anticonceptivos hormonales o terapia hormonal. Puede afectar a todos los grupos de edad, desde la adolescencia hasta la posmenopausia. La mayor prevalencia se registra en los extremos de la vida reproductiva, particularmente en la adolescencia y la perimenopausia, períodos que se caracterizan por una mayor concentración de ciclos anovulatorios o irregulares. Consideraciones finales: La historia clínica debe orientarse a identificar la naturaleza del sangrado, identificar posibles causas estructurales u orgánicas, impactar en la calidad de vida y evaluar las expectativas de la mujer, especialmente en cuanto a la necesidad de anticoncepción o deseo de embarazo.Introdução: O sangramento menstrual acontece como um evento endometrial universal, autolimitado, que se segue à queda dos níveis hormonais (supressão de estrogênio e progesterona) em um ciclo ovulatório normal. O mecanismo é complexo e ordenado, de forma que a desregulação dos eventos moleculares, celulares e vasculares sequenciais envolvidos pode levar à acentuada variedade de distúrbios menstruais. Objetivo: elucidar as principais causas do sangramento uterino anormal, bem como suas etiologias. Metodologia: trata-se de uma revisão narrativa de literatura. A pesquisa foi realizada através do acesso online nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultados e discussão: A denominação “Sangramento Uterino Anormal” (SUA) inclui sangramento disfuncional (anovulatório ou ovulatório) e sangramento por causas estruturais (miomas, pólipos, adenomiose, adenomiomas, carcinoma endometrial, complicações da gravidez). O SUA pode ainda estar relacionado ao uso de contraceptivos hormonais ou terapia hormonal. Pode acometer todas as faixas etárias, desde a adolescência até a pós-menopausa. As maiores prevalências são registradas nos extremos da vida reprodutiva, particularmente na adolescência e perimenopausa, períodos que se caracterizam por uma concentração maior de ciclos anovulatórios ou irregulares.
