Closure of buccosinusal communication with bichat ball: case report

Resumo: Buccosinusal communication occurs after extraction of upper molars due to its anatomical proximity to the maxillary sinus, characterized by the access between the oral cavity and the maxillary sinus due to the defect in bone and soft tissue. The objective was to present a case report of closure of sinus buccal communication with bichat ball. Male patient, 54 years old, attended the Solon Galvão Clinic (Universidade Potiguar - UnP, Natal-RN, Brazil) presenting a sinus buccal communication on the right side of the maxilla resulting from an extraction of the element 17. A maxilla cone bean computed tomography was requested, where it was proven that during the extraction of the element 17, the anterior wall of the maxillary sinus was also removed, causing communication. The maxillary sinus was cleaned, removing inflammatory cysts from inside the cavity. Then, there was exposure and mobilization of the bichat ball, followed by suturing it around the lesion in the mucosa and suture of the mucosa over the bichat ball, using absorbable thread. It is concluded that it is a safe technique, easy to perform, benefiting a satisfactory result for the patient.

It is concluded that it is a safe technique, easy to perform, benefiting a satisfactory result for the patient.La comunicación buco-sinusal se produce tras la extracción de los molares superiores por su proximidad anatómica al seno maxilar, caracterizada por el acceso entre la cavidad bucal y el seno maxilar debido al defecto en hueso y tejido blando. El objetivo fue presentar un reporter de caso de cierre de comunicación sinusal bucal con bichatt ball. Paciente masculino de 54 años que acudió a la Clinica Solon Galvão (Universidade Potiguar – UnP, Natal-RN, Brasil) que presenta una comunicación sinusal bucal en el lado derecho del maxilar como resultado de la extracción del elemento 17. Se solicitó tomografia computarizada de frijol cono maxilar, donde se comprobó que durante la extracción del elemento 17 también se removió la pared anterior del seno maxilar, provocando comunicación. Se limpió el seno maxilar, removiendo los quistes inflamatorios del interior de la cavidad. Posteriormente, se expuso y movilizó la bola de bichat, seguida de sutura alrededor de la lesión en la mucosa y sutura de mucosa sobre la bola de bichat, utilizando hilo reabsorbible. Se concluye que es una técnica segura, fácil de realizar, beneficiando un resultado satisfactorio para el paciente.A comunicação bucosinusal ocorre após a exodontia de molares superiores por sua proximidade anatômica ao seio maxilar, caracterizando pelo acesso entre a cavidade oral e o seio maxilar devido ao defeito em tecido ósseo e tecido mole. Objetivou-se apresentar um relato de caso de fechamento de comunicação buco sinusal com bola de bichat. Paciente masculino, 54 anos, compareceu a Clínica Solon Galvão (Universidade Potiguar – UnP, Natal-RN, Brasil) apresentando uma comunicação buco sinusal em lado direito da maxila decorrente de uma exodontia do elemento 17.  Foi solicitada uma tomografia computadorizada cone bean de maxila, onde foi comprovada que durante a exodontia do elemento 17, foi retirado também a parede anterior do seio maxilar, causando a comunicação. Foi feito a limpeza do seio maxilar, retirando cistos inflamatórios de dentro da cavidade. Após, houve exposição e mobilização da bola de bichat, seguido de sutura da mesma ao redor da lesão em mucosa e sutura da mucosa por cima da bola de bichat, utilizando fio absorvível.
