Application of wound dressings based on bacterial cellulose in the healing of chronic and complex wounds: Trends and perspectives

Resumo: The aim of this study was to carry out a narrative literature review of the most recent studies and technological advances related to the biomedical applications of dressings composed of bacterial cellulose biopolymer (BC) in the healing of chronic and complex wounds. BC can be produced on a large scale through a fermentation process by specific bacteria. Cellulose fibrils form a highly porous network, similar to the structure of the extracellular matrix of tissues, as well as having favorable physicochemical characteristics for applications as dressings. Recent studies have shown the importance of and interest in incorporating active pharmaceutical ingredients of both synthetic and natural origin, with the aim of conferring specific pharmacological properties to the structure of these polymeric matrices. The physicochemical versatility of BC allows this material to be functionalized and modified through various techniques, both ex situ and in situ, to obtain new hybrid materials with functional properties for different biomedical applications. Among the pharmaceutical active ingredients incorporated into a CB matrix for application to chronic and complex wounds are antiseptic and antimicrobial active ingredients, anti-inflammatories from natural sources and growth factors. Another strategy that has proved promising is the application of mesenchymal stem cells and components of their secretome in the polymeric matrix of the BS. Therefore, this review sheds light on the latest technological advances regarding the different ways of functionalizing BC-based dressings for the treatment of chronic and complex wounds.

La BC puede producirse a gran escala mediante un proceso de fermentación por bacterias específicas. Las fibrillas de celulosa forman una red muy porosa, similar a la estructura de la matriz extracelular de los tejidos, además de tener características fisicoquímicas favorables para su aplicación como apósitos. Estudios recientes han puesto de manifiesto la importancia y el interés de incorporar principios activos farmacéuticos, tanto de origen sintético como natural, con el objetivo de conferir propiedades farmacológicas específicas a la estructura de estas matrices poliméricas. La versatilidad fisicoquímica del CB permite funcionalizar y modificar este material mediante diversas técnicas, tanto ex situ como in situ, para obtener nuevos materiales híbridos con propiedades funcionales para diferentes aplicaciones biomédicas. Entre los principios activos farmacéuticos incorporados a la matriz de CB para su aplicación en heridas crónicas y complejas se encuentran principios activos antisépticos y antimicrobianos, antiinflamatorios de origen natural y factores de crecimiento. Otra estrategia que ha resultado prometedora es la aplicación de células madre mesenquimales y componentes de su secretoma en la matriz polimérica de la EB. Así pues, esta revisión arroja luz sobre los últimos avances tecnológicos en relación con las distintas formas de funcionalizar los apósitos a base de CB para el tratamiento de heridas crónicas y complejas.Este estudo teve como objetivo realizar uma revisão narrativa da literatura dos mais recentes estudos e avanços tecnológicos relacionados às aplicações biomédicas de curativos compostos por biopolímero de celulose bacteriana (CB) na cicatrização de feridas crônicas e complexas. A CB pode ser produzida em larga escala através de processo de fermentação por bactérias específicas. As fibrilas de celulose formam uma rede altamente porosa, semelhante à estrutura da matriz extracelular de tecidos, além de possuírem características físico-químicas favoráveis para aplicações como curativos. Recentes estudos têm demonstrado a importância e o interesse na incorporação de ativos farmacêuticos tanto de origem sintética quanto natural, com objetivo de conferir propriedades farmacológicas específicas à estrutura destas matrizes poliméricas. A versatilidade físico-química da CB permite que este material seja funcionalizado e modificado através de diversas técnicas, ex situ e in situ, para obter novos materiais híbridos e com propriedades funcionais para diferentes aplicações biomédicas. Dentre os ativos farmacêuticos incorporados em matriz de CB para aplicação em feridas crônicas e complexas, destacam-se os ativos antissépticos e antimicrobianos; os antiinflamatórios de fontes naturais, bem como os fatores de crescimento. Outra estratégia que tem se demonstrado promissora é a aplicação de células-tronco mesenquimais e componentes de seu secretoma na matriz polimérica da CB.
