Characterization of the family experience of children and adolescents with Diabetes mellitus type 1: a narrative review

Resumo: Diabetes Mellitus (DM) is a chronic health condition characterized by hyperglycemia resulting from the impossibility of production or deficiency in the secretion or action of insulin. Type 1 DM is the most frequent in children and adolescents. This study aims to investigate characteristics of family life after the diagnosis of type 1 diabetes mellitus in children and adolescents and the possible difficulties encountered after this period. This is a narrative review, with a search for papers in the Latin American and Caribbean Literature in Health Sciences (Lilacs), Pubmed/Medline, Scientific Electronic Library Online (Scielo) and Cochrane databases with studies published in the last 20 years. It is evidenced by the studies that the discovery of the disease impacts the routine and changes in the family lifestyle, showing as the main difficulties the application of insulin and proposed dietary adaptations, added to feelings such as guilt, fear, anxiety and frustration. In these circumstances, the support of guardians is essential for the acceptance and control of the disease by the child or adolescent. The changes evidenced can cause psychological and emotional changes, as well as in the well-being and in the proper course of the disease, thus suggesting the need for support and guidance focused on improving the quality of life of children and adolescents and their families.

La DM tipo 1 es la más común en niños y adolescentes. Éste estudio tiene como objetivo investigar las características de la vida familiar después del diagnóstico de diabetes mellitus tipo 1 en niños y adolescentes y las posibles dificultades encontradas después de este período. Se trata de una revisión narrativa, con búsqueda de artículos en las bases de datos de Literatura Latinoamericana y Caribeña en Ciencias de la Salud (Lilacs), Pubmed / Medline, Scientific Electronic Library Online (Scielo) y Cochrane con estudios publicados en los últimos 20 años. Los estudios evidencian que el descubrimiento de la enfermedad impacta en la rutina y cambios en el estilo de vida familiar, mostrando como principales dificultades la aplicación de insulina y las adaptaciones dietéticas propuestas, sumadas a sentimientos como la culpa, el miedo, la ansiedad y la frustración. En estas circunstancias, el apoyo de los tutores es fundamental para la aceptación y control de la enfermedad por parte del niño, niña o adolescente. Los cambios evidenciados pueden ocasionar cambios psicológicos y emocionales, así como en el bienestar y en el curso adecuado de la enfermedad, sugiriendo la necesidad de apoyo y orientación enfocados a mejorar la calidad de vida de los niños, niñas y adolescentes y sus familias.O Diabetes Mellitus (DM) é uma condição crônica de saúde caracterizada por hiperglicemia decorrente da impossibilidade de produção ou deficiência na secreção ou ação da insulina. O DM tipo 1 é o mais frequente em crianças e adolescentes. Este estudo tem como objetivo investigar características da vivência familiar após o diagnóstico de diabetes Mellitus tipo 1 em crianças e adolescentes e as possíveis dificuldades encontradas após esse período. Trata-se de uma revisão narrativa, com busca de trabalhos nas bases de dados Literatura Latino Americana e do Caribe em Ciências da Saúde (Lilacs), Pubmed/ Medline, Scientific Eletronic Library Online (Scielo) e Cochrane com estudos publicados nos últimos 20 anos. É evidenciado pelos estudos que a descoberta da doença causa impacto na rotina e alterações no estilo de vida familiar, mostrando-se como principais dificuldades a aplicação de insulina e as adequações alimentares propostas, acrescida de sentimentos como culpa, medo, ansiedade e frustração. Nessas circunstâncias, o apoio dos responsáveis mostra-se fundamental para aceitação e controle da doença pela criança ou adolescente.
