Nutritional approach in people diagnosed with COVID-19 hospitalized in Intensive Care Units: an integrative review

Resumo: COVID-19 is a disease caused by the viral agent SARS-CoV-2, which began in 2019 and became a pandemic in 2020. It affected individuals around the world, causing changes since the routine life of populations to serious injuries to those infected. Of these, some require hospitalization in Intensive Care Units due to the severity of the disease. Therefore, in order for the inflammation, hypermetabolism and gastric alterations imposed by the disease to be minimized, it is important to adopt an effective nutritional therapy, which is possible from the identification of the individual at nutritional risk, the establishment of a nutritional diagnosis and the adoption of nutritional therapy. Thus, the objective was to consolidate the data available in the literature on the screening and nutritional assessment tools used in ICUs, as well as the nutritional therapy used in these patients. The methodology was based on the analysis of articles taken from PubMed, SciELO, MedLine and Google Scholar using the Health Sciences Descriptors. Twelve articles that met the criteria were selected. According to the findings in the studies, it is observed that the NRS-2002 is a highly recommended and validated screening tool, as are the subjective global assessment (SGA) and GLIM for nutritional assessment. In addition, it is noted that nutritional therapy follows existing recommendations, but with specificities regarding specific supplements for the disease. In short, the data obtained to date are sufficient for efficient nutritional management, despite the need to develop more studies involving critically ill patients with COVID-19.

Afectó a personas de todo el mundo, provocando cambios desde la vida cotidiana de las poblaciones hasta lesiones graves en los infectados. De estos, algunos requieren hospitalización en Unidades de Cuidados Intensivos debido a la gravedad de la enfermedad. Por lo tanto, para minimizar la inflamación, el hipermetabolismo y las alteraciones gástricas que impone la enfermedad, es importante adoptar una terapia nutricional eficaz, que es posible a partir de la identificación del individuo en riesgo nutricional, el establecimiento de un diagnóstico nutricional y la adopción de la terapia nutricional. Así, el objetivo fue consolidar los datos disponibles en la literatura sobre las herramientas de cribado y valoración nutricional utilizadas en las UCI, así como la terapia nutricional empleada en estos pacientes. La metodología se basó en el análisis de artículos tomados de PubMed, SciELO, MedLine y Google Scholar utilizando los Descriptores de Ciencias de la Salud. Se seleccionaron doce artículos que cumplían con los criterios. De acuerdo con los hallazgos de los estudios, se observa que la NRS-2002 es una herramienta de tamizaje altamente recomendada y validada, al igual que la evaluación subjetiva global (ESG) y la GLIM para la evaluación nutricional. Además, se destaca que la terapia nutricional sigue las recomendaciones existentes, pero con especificidades en cuanto a suplementos específicos para la enfermedad. En definitiva, los datos obtenidos hasta el momento son suficientes para un manejo nutricional eficiente, a pesar de la necesidad de desarrollar más estudios con pacientes críticos con COVID-19.A COVID-19 é uma doença causada pelo agente viral SARS-CoV-2, que teve seu início no ano de 2019 e passou a ser considerada uma pandemia no ano de 2020. Esta, afetou indivíduos em todo o mundo, causando alterações desde a rotina de vida das populações até lesões sérias nos infectados. Desses, alguns necessitam de internação em Unidades de Terapia Intensiva devido à gravidade da doença. Portanto, para que o quadro de inflamação, hipermetabolismo e alterações gástricas impostas pela doença sejam minimizados, faz-se importante a adoção de uma terapia nutricional eficaz, que é possível a partir da identificação do indivíduo em risco nutricional, o estabelecimento do diagnóstico nutricional e a adoção da terapêutica nutricional. Assim, objetivou-se realizar uma consolidação dos dados disponíveis na literatura sobre as ferramentas de triagem e de avaliação nutricional utilizadas nas UTIs, bem como a terapia nutricional utilizada nesses pacientes. A metodologia pautou-se na análise de artigos retirados do PubMed, SciELO, MedLine e Google Acadêmico utilizando os Descritores em Ciências da Saúde. Foram selecionados 12 artigos que cumpriram os critérios. De acordo com os achados nos estudos, observa-se que a NRS-2002 é uma ferramenta de triagem bastante recomendada e validada, assim como a GLIM para a avaliação nutricional. Ademais, nota-se que a terapia nutricional segue recomendações já existentes, mas com especificidades quanto a suplementações específicas para a doença.
